三维适形放疗联合顺铂和足叶乙甙同期及巩固化疗治疗局部晚期非小细胞肺癌临床观察

摘　　要：目的探讨三维适形放射治疗联合顺铂和足叶乙甙同期及巩固化疗治疗局部晚期非小细胞肺癌的临床疗效及不良反应。方法将58例局部晚期非小细胞肺癌(IIIA-N2或IIIB期)患者随机分入三维适形放射治疗联合顺铂和足叶乙甙同期及巩固化疗组(实验组,n=29)、三维适形放射治疗联合顺铂和足叶乙甙同期化疗组(对照组,n=29)。两组均用6-MV线,常规剂量分割,三维适形放疗,总剂量66-70Gy;同期及巩固化疗为EP方案(E:依托泊苷60mg/m2,1-5d;P:顺铂20mg/m2,1-5d)。结果实验组和对照组的有效率分别为69%、62.1%,两组比较差异无统计学意义(P>0.05);实验组和对照组1、2、3年生存率分别为79.3%、62.1%、48.3%和75.9%、44.8%、20.7%,3年生存率比较差异有统计学意义(P<0.05);主要毒性为骨髓抑制、放射性肺炎和放射性食管炎,但两组并未出现明显差异。结论三维适形放射治疗联合顺铂和足叶乙甙同期及巩固化疗较同期放化疗治疗局部晚期非小细胞肺癌患者有较好的疗效,且患者能耐受。Objective To explore the clinic efficacy and side effects of three dimensional conformal radiotherapy （3-DCRT） combined with cisplatin and etoposide concurrent or consolidation chemotherapy for locally advanced non-small cell lung cancer （LANSCLC）. Methods 58 patients with LANSCLC（IIIA-N2、IIIB stage）were randomly divided into concomitant chemoradiotherapy plus consolidation （experiment group） and concomitant radiochemotherapy （control group）. The two groups were given by 6-MV-X ray,routine dosage fraction,3-DCRT,the total dosage of 66-70Gy; the two groups were given cisplatin and etoplside （E：etoposide 60mg/m2,d1-5;P：cisplatin 20mg/m2,d1-5）,concurrent and consolidation chemotherapy were two cycles respectively.Results The response rate in experiment group and control group was 69%、62.1%,with no statistic significance; the one year,two year,three year survival rates were 79.3%,62.1%,48.3% and 75.9%,44.8%,20.7% respectively in two groups,with statistic difference（P0.05）; the side-effects were mainly myleosuppression,radiation pneumonitis and radiation esophagitis,with no statistic differences.Conclusion Three dimensional conformal radiotherapy combined with cisplatin and etoposide concurrent or plus consolidation chemotherapy for locally advanced non-small cell lung cancer was well tolerated,and the better efficacy in experiment group than control group.
