摘　　要：目的:分析肝血管平滑肌脂肪瘤(angiomyolipoma,AML)的螺旋CT(ScrewCT,SCT)征象,探讨其SCT征象特征。方法:回顾性分析28例经手术病理证实的AML患者平扫和三期增强扫描的SCT征像。结果:28例中14例(50%)为肝混合型AML表现,平扫病灶由稍低密度软组织成分和低密度脂肪成分构成,边缘清楚。3例病灶为脂肪型AML表现,14例病灶为平滑肌型或血管型AML表现,增强扫描13例25个病灶动脉期软组织成分均明显强化;门脉期15个病灶密度高于周围肝实质;延迟期仍为高密度者8个。增强扫描病灶的脂肪成分均不同程度强化。12例的24个病灶周边出现扭曲血管与肝动脉相连;门脉期和实质期有9个病灶周围可见包膜样强化。7个病灶周围可见向肝静脉引流的引流静脉。结论:SCT可以展现肝血管平滑肌脂肪瘤的影像特征,对术前诊断有较高的价值。Objective：Analyse liver blood vessel smooth muscle lipoma （angiomyolipoma,the AML） helix CT （Screw CT,SCT） solicits an elephant,discuss whose SCT sign characteristic.Methods：Retrospect sex analyses 28 examples collecting an image by that the AML patient tie that operation pathological mechanism confirms sweeps SCT strengthening scanning with three phases.Results：（50%） shows 14 examples of 28 example middle for liver mixed type AML,flat sweep nidus from slightly low-density soft tissue component and low-density fat component formation,border are clear.AML shows 3 example nidus for fat type,AML shows,strengthens 14 example nidus for the smooth muscle type or the blood vessel type to scan 13 examples 25 nidus the arterial scheduled time soft tissue component intensifies without exception obviously;Door pulse expects 15 nidus density is higher than vicinity liver essence;Postpone still being 8 high density person phase.The component strengthens the fat strengthening scanning nidus without exception to varying degrees.24 of 12 examples nidus dogleg blood vessel and arteria hepatica appears on periphery being linked together;Door pulse scheduled time and essence scheduled time have 9 the visible bag sample of film intensifies around nidus.7 nidus visible vicinity drainage vein to vena hepatica drainage.Conclusion：SCT can display the portrait of liver blood vessel smooth muscle lipoma characteristic,has comparatively high value to diagnosing in the skill front.
