保留幽门的近全胃切除回盲肠间置代胃术的初步报告

摘　　要：目的：观察保留幽门近全胃切除回结肠间置代胃术在近端胃癌外科治疗中的疗效。方法；对5例近端胃癌患者行保留幽门近全胃切除回结肠间置代胃术，术后行影像学、同位素99mTc－DTPA闪烁照像检查评价代胃功能。结果：间置回盲肠段具有食物贮存、良好的排空、抗返流功能。近期随访无返流性食管炎、倾倒综合征。结论：该术式用于近端胃癌有助于预防术后并发症和改善患者生存质量。To observe curative effect of interpositional ileocaecogastroplasty after pylorus-preserving near-total gastrectomy. Method: Ileocaecogastroplasty reconstruction after pylorus-preserving near-total gastrec-tomy was performed in 5 cases of proximal gastric cancer. The function of substituted stomach was evaluated by radiological and postoperation 99mTc-DTPA scintigraphic examinations. Result: Interpositional ileocaecal segment had a good capaclty of gastric-emptying, food-reserving and anti-reflux. Dumping syndrome and reflux oe-sophagtitis were not observed in all patients in short-term follow-up studies. Conclusion: This new technique can be used in proximal gastric cancer, and it is helpful to prevent postoperative complications and improve life quali-ty of the patients.
