腔镜辅助下伴不全肠梗阻的右半结肠癌根治术临床分析

摘　　要：目的为探讨腔镜辅助下伴不全肠梗阻的右半结肠癌根治术的手术及围手术期治疗的相关问题。方法回顾性分析该院进行腔镜辅助下伴有不全肠梗阻的右半结肠癌根治术的30例患者的手术及围手术期处理。结果 30例患者均成功施术,手术时间(130±20)min,术中出血(110±55)mL,术后平均住院时间(7±2.5)d,29例患者无术后并发症,1例患者出现术后腹腔脓肿,经腹腔持续冲洗引流治愈。术后均施以化疗。随访0.5～3年,无术后肿瘤复发。结论伴不全肠梗阻的右半结肠癌具有自身独特特点,只要注意围手术期的处理及术中灵活运用腔镜及开腹操作,腔镜辅助下治疗伴不全肠梗阻的右半结肠癌是安全可行的。【Objective】To research related problems about laparoscopic right hemicolon＇s cancer with pero-obstruction radical resection. 【Method】The operative method and treatment of operation＇s circa about 30 cases of laparoscopic right hemicolon＇s cancer with pero-obstruction radical resection were retropectively analyzed.【Result】 30 cases were performed laparoscopic radical operation. The meantime of operation was （130±20） min,the bleeding was （110±55） mL,the time in hospital of postoperation was （7±3.5） d. 29 patients were recovered without complication. 1 cases appeared abdominal abscess. After abdominal lavage and drainage,the patients recovered. No patients appeared recurrence of tumor during the follow-up during 3 years to half year. 【Conclusion】The right hemicolon＇s cancer with pero-obstruction has its unique features. If we can well handle the related problems of operation and treatment of operation＇circa,the laparoscopic right hemicolon＇s cancer with pero-obstruction radical resection is safe and feasible.
