摘　　要：目的探讨中枢神经细胞瘤(CNC)的MRI表现。资料与方法搜集临床资料完整并经病理证实的CNC患者11例(年龄17～52岁,平均33岁)。10例患者同时行MR平扫及增强扫描,1例左侧小脑半球CNC患者仅行MR增强扫描。结果 11例CNC中10例位于侧脑室前部Monro孔附近,1例位于左侧小脑半球。MR平扫10例侧脑室内CNC均表现信号明显不均一,内见多发囊变区,3例伴少量出血;T1WI呈不均匀等/低信号,3例病灶内混杂斑片状高信号;T2WI呈不均匀等/高信号,混杂斑片状稍高信号及低信号;增强扫描9例呈斑片状、条索状不均匀明显强化,1例呈小片状轻度强化。左侧小脑半球CNC表现为大囊性病灶,内见附壁小结节明显强化。侧脑室CNC均显示病灶条索状、幕状粘连于侧脑室壁,尤其在MRI矢状位显示最佳。结论位于侧脑室前部Monro孔附近信号不均一的肿块,不均匀斑片状明显强化或轻度强化,条索状、幕状粘连于侧脑室壁,是中央性CNC的典型MRI表现。脑室外CNC可表现为大囊性病灶,内见附壁小结节明显强化。Objective To explore the typical MR appearances of central neurocytoma.Materials and Methods Eleven patients（ages ranging from 17 to 52 years,mean age of 33 years） with central neurocytoma confirmed by pathology were collected in the study.Ten patients underwent conventional and contrast MRI and one patient with central neurocytoma located in left cerebellar hemisphere underwent contrast MRI only.The MR appearances of central neurocytoma were analyzed retrospectively.Results Ten central neurocytomas located in anterior lateral ventricle adjacent to the Monro hole,another tumor located in left cerebellar hemisphere.All the 10 central neurocytomas located in lateral ventricle showed as heterogeneous signal intensity on conventional MRI,especially on T2 weighted images.Two lesions displayed punctiform hyper-intensity inside the tumors on T1 weighted images.On contrast MR images,9 lesions displayed heterogeneous significant enhancement and 1 lesions displayed heterogeneous slight enhancement.However,another lesion located in left cerebellar hemisphere demonstrated big cyst and small nodule adhere to cyst wall with significant enhancement.All the 10 central neurocytomas located in lateral ventricle adhered to the walls of the lateral ventricle with the tentiform and cordlike pattern,especially on sagittal MR images.Conclusion The typical MR appearances of centric central neurocytoma included the mass located in the anterior lateral ventricle adjacent to the Monro hole,heterogeneous signal intensity and punctiform significant enhancement,adhere to the walls of the lateral ventricle with the tentiform and cordlike pattern.Extraventricular neurocytoma located in cerebellar hemisphere might demonstrate significant enhancement small nodule adhere to the big cyst.
