摘　　要：目的分析髌骨肿瘤的病种分布及其影像学表现。资料与方法回顾性分析本院自1976年至2008年经病理证实的髌骨肿瘤患者资料,分析髌骨骨肿瘤的发病情况及影像学表现。结果髌骨肿瘤共26例,男女比例17∶9。以软骨母细胞瘤(8例)最为多见,其次为骨巨细胞瘤(4例),髌骨的恶性肿瘤少见。影像学表现中,软骨母细胞瘤、骨巨细胞瘤,甚至纤维结构不良均可表现为膨胀性生长,有时难以区分。但软骨母细胞瘤倾向于病灶小,边缘硬化,有时见病灶内钙化。结论髌骨肿瘤以良性为主,恶性肿瘤少见;综合分析髌骨不同肿瘤的发病率及其影像特征,有助于提高髌骨肿瘤的诊断正确率。Objective To analyze the distribution and imaging features of tumor and tumor like lesion in patellar.Materials and Methods The cases of histologically proven tumor and tumor-like lesions in patellar were collected.The distribution and imaging features of these lesions were analyzed.Results There were 26 cases of patellar tumors and tumor-like disease,the male to female ratio was 17：9.In all of these cases,benign lesions were 69.2%（18 cases） and malignant tumors were 30.8%（8 cases）.Chondroblastoma（8 cases） was the most common tumor invloved patella,followed by giant cell tumor（4 cases）.Malignant tumors in patellar were rare,of which mainly were metastatic lesions and osteosarcoma.Imaging findings in chondroblastoma,giant cell tumor of bone,and even fibrous dysplasia presented as expansive bone destruction,and hard to distinguish from each other.Chondroblastoma usually showed as small tumor with marginal sclerosis or calcification in the lesion.Conclusion The patellar tumors are mainly benign lesions.Comprehensive understanding of the distribution,incidence,imaging features of patellar tumors can improve the diagnosis accuracy.
