经腹腔镜D2远端胃癌根治术33例临床观察

摘　　要：目的：探讨经腹腔镜D2远端胃癌根治术手术操作的方法及近期效果。方法：对33例进展期胃癌经腹腔镜行D2远端胃癌根治术。蛄果：33例均成功进行腹腔镜手术。手术平均用时（290±43）min。术中平均出血量（154±85）ml，平均清扫淋巴结（20．79±5．21）枚。术后病人平均胃肠功能恢复时间（50±24）h，下床活动时间（55±20）h，进食时间（68±24）h，术后近期生活质量良好。结论：经腹腔镜D2远端胃癌根治术安全、可行，与开腹手术的根治效果相当，且有创伤小、术后康复快等优点。Objective：To discuse the operative approaches and clinical outcomes of laparoscopy - assisted distal gastrectomy and D2 lymph node dissection. Methods： A total of 33 patients with gastric cancer were performed distal gastrectomy. Results： Of the 33 patients, all were performed laparoscopic - assisted distal surgery successfully. The average operative time was （ 290 ± 43 ） minutes. The average blood loss was （ 154 ± 85 ） ml. The average number of the harvested lymph nodes was （20. 79± 5.21 ）. The average time for gastrointestinal function recovery, for the patients＇ standing up and for the start of oral liquid taking was（50 ±24）, （55 ±20） and （68 ±24） hours after operation,respectively. The patients＇ recent recovery and living quality were good. Conclusion：Laparoscopy-assisted radical gastrectomy is safe ,feasible, minimally invasive and can achieve the same result of abdom inal opening operation.
