血清CA125、CA72-4联合超声血流检测在卵巢上皮恶性肿瘤诊断中的价值

摘　　要：目的:探讨血清肿瘤标志物CA125、CA72-4与超声血流联合检测对卵巢上皮恶性肿瘤的诊断价值。方法:选择49例卵巢上皮恶性肿瘤患者和52例卵巢良性上皮肿瘤患者,用放射免疫法测量血清CA125、CA72-4水平,并行彩色多普勒超声检查肿瘤内部血流分布情况,测量血流阻力指数(RI)、搏动指数(PI),并将诊断结果与病理结果进行对照。结果:血清CA125、CA72-4、RI、PI单检对卵巢上皮恶性肿瘤诊断的灵敏度分别为75.5%、65.3%、77.6%、73.5%;CA125+CA72-4、RI+PI、CA125+CA72-4+RI、CA125+CA72-4+RI+PI联检对卵巢上皮恶性肿瘤诊断的灵敏度分别为87.8%、83.7%、89.8%、95.9%,联检诊断敏感性提高,与单检敏感性比较差异有统计学意义(P<0.05)。联检的准确度与诊断价值也较单检高。结论:血清CA125、CA72-4与超声血流检测联合应用能提高对卵巢上皮恶性肿瘤诊断的敏感性及准确性,能成为早期诊断卵巢上皮恶性肿瘤较好的方法。Objective：To investigate the clinical value of combined measurement of serum CA125, CA72-4, color Doppler flow imaging for the diagnosis of epithelial ovarian cancer.Methods：A total of 101 patients with epithelial ovarian tumor were selected, including forty-nine cases of epithelial ovarian cancer, 52 epithelial ovarian benign tumour.The levels of serum CA125, CA72-4 were measured by radio-immunity analysis.Color Doppler were used to research the distribution of blood flow of the tumor, resistant index （RI） and pulsation index （PI） were measured by color Doppler.All the diagnose results of CA125, CA72-4, RI and PI were compared to the clinical pathology.Results：The diagnose sensitivities of epithelial ovarian cancer by CA125, CA72-4, RI and PI were 75.5%, 65.3%, 77.6%, 73.5%.The diagnose sensitivities of combined measurements such as CA125＋CA72-4, RI＋PI, CA125＋CA72-4＋RI, CA125＋CA72-4＋RI＋PI were 87.8%, 83.7%, 89.8%, 95.9%. The sensitivities of combined measurements were obviously higher than that of the single measurement （P〈0.05）.The diagnose accuracy and clinical value of combined measurements were higher than those of single measurement too.Conclusion：Combined measurements of CA125, CA72-4 in serum, RI and PI can improve diagnostic sensitivity and accuracy, which can be a good method to diagnose the epithelial ovarian cancer.
