万拉法辛治疗精神分裂症阴性症状的临床研究

摘　　要：目的探讨抗抑郁剂万拉法辛治疗精神分裂症阴性症状的临床疗效和安全性。方法将60例以阴性症状为主的精神分裂症患者随机分为实验组和对照组,分别给予利培酮合用万拉法辛及单用利培酮治疗,疗程12周,于治疗前、治疗4、8和12周末用阳性和阴性症状量表(PANSS)评定其疗效,用副反应量表(TESS)评定其药物的安全性。结果治疗后4周实验组PANSS总分、阳性因子分、阴性因子分、一般精神病理分以及对照组PANSS总分、阳性因子分与疗前比较,差异有统计学意义(t=2.02～2.20,P<0.05);治疗后8、12周2组PANSS总分和各因子分与疗前比较,差异有统计学意义(t=3.45～20,P<0.01)。治疗后4、8、12周2组间比较阴性因子分差异有统计学意义(t=4.44～11,P<0.01)。2组药物副反应(TESS)在治疗4、8和12周末比较差异无统计学意义。结论在利培酮基础上联用万拉法辛治疗精神分裂症阴性症状起效较快,显示良好的临床疗效,药物不良反应不明显,可以作为以阴性症状为主的精神分裂症的治疗康复用药。[ Objective] To study the efficacy and safety of venlafaxine in treatment of negative symptoms of schizophrenia. [ Methods] A total of 60 cases of schizophrenia with prominent negative symptoms were randomly divided into experimental group and con- trol group, treated with risperidone combined with venlafaxine, and single risperidone, respectively, for 12 weeks. The PANSS and TESS were employed for assessing the therapeutic efficacy and safety respectively, before treatment and g, 8, 12 weeks after treatment. [ Results ] After treatment, the total scores of PANSS, positive factor scores, negative factor scores, general psychopathology scores of experimental group and the total scores of PANSS, positive factor scores of control group were significantly different respectively, compared with those before treatment （ t = 2.02-2.20, P 〈 0.05 ）, as well as the total scores of PANSS and factors scores of 2 groups after 8, 12 weeks treatment （t =3.45-20,P 〈0.01 ） The difference of negative factor scores between 2 groups was significant （ t =4.44-11 ,P 〈0. 01 ） 4, 8, 12 weeks after treatment. The scores of TESS had no significant difference between 2 groups 4, 8, 12 weeks after treatment. [ Conclusion] Risperidone combined with venlafaxine has rapid and excellent efficacy in treatment of negative symptoms of schizophrenia, without obvious adverse reactions, can be used for the medication of schizophrenia with predominantly negative symptoms.
