高低不同责任状态下强迫症患者项目记忆及源记忆的研究

摘　　要：目的比较强迫症(OCD)患者和正常人的源记忆及项目记忆,以及OCD患者在不同责任状态下源记忆与项目记忆的差异;检验源记忆与项目记忆成绩和强迫症状是否相关。方法创设高低不同责任条件,测试35例OCD患者和35例正常人的项目记忆及源记忆任务。对OCD患者组进行YALE-BROWN强迫量表评分。结果与正常对照相比,OCD患者的项目记忆和源记忆任务成绩差异均有统计学意义(P<0.05)。与低责任任务相比,高责任任务中的OCD患者的项目记忆成绩差异有统计学意义(P<0.01),但源记忆差异没有统计学意义(P=0.63)。与低责任任务相比,责任感态度问卷(RAS)高得分组OCD患者的项目记忆和源记忆任务成绩差异均有统计学意义(P<0.05)。项目记忆和源记忆的损害程度与强迫症状不相关。结论 OCD患者的项目记忆和源记忆均有一定程度的损害。高责任任务状态下,OCD患者的记忆成绩影响更加显著。对那些体会到过多责任感的患者,项目记忆和源记忆的损害程度更加明显。Objective To compare the effect of source memory and item memory in OCD patients and normal,in the different responsibility conditions,and to test whether the impairment of item memory and source memory has correlation to the grades of obsessive-compulsive symptoms. Methods Establishing the high and low experimental conditions,we tested participants including 35 OCD patients and 35 normal persons in item memory task and source memory task. The YALE-BROWN was employed for assessing the symptoms of OCD patients. Results The performances of item memory and source memory in OCD patients were significantly lower than that in normal control （ P 0. 05） . The performance of item memory in OCD patients in the high responsibility condition were significantly lower than that in the low responsibility condition（ P 0. 01） ,but this performance did not appear in source memory （ P =0. 63） . The performances of item memory and source memory in OCD patients of high score group in RAS in the high responsibility condition were significantly lower than that in the low responsibility condition（ P 0. 05） . The impairment of item memory and source memory had no correlation to the grades of obsessive-compulsive symptoms. Conclusion Item memory and source memory in OCD patients have impairment compared with normal. Memory performance of OCD patients has been found to be more affected in the high responsibility condition. Memory performance of the OCD patients who perceive more responsibility has manifestd more obvious impairment.
