传导性角膜成形术矫治老视对立体视的影响

摘　　要：目的对比分析传导性角膜成形术（CK）手术前后各时期的视力，远、近立体视锐度，评估CK手术对患者立体视的影响。方法对42例（55只眼）老视症状者按monovision方式设计，非主视眼手术以改善近视力，其中13例伴轻度远视者另一只眼同时手术以提高远视力，分别于术前、术后1、3月和6月测定远、近立体视锐度。结果术后6个月患者双眼裸眼视力达0．8／0．33（J4）以上者有（90％）38／42，而术前只有（10％）4／42的患者有此视力。术后1个月时近立体视较术前有减退趋势，但无低于100”者，随时间延长逐渐改善，至术后3个月时已优于术前。术后1个月时远融合范围减少及远立体视的降低，差异无统计学意义（Х^2融=0．45，Х^2远=5．79，P〉0．05），术后3个月时接近术前水平。结论CK术后早期远、近立体视均无明显下降。近立体视术后恢复快且优于术前。Objective To analyze distant and near vision, stereoacuity before and after CK, evaluate the effect of CK on stereopsis. Methods One eye of 42 cases was operated CK to improve near vision, and the other eye of 13 cases with hyperopia of them was operated to improve distant vision. Otherwise, the acuity of distant and near stereopsis was measured at preoperative, 1month, 3month, and 6month after operation. Results There were 90% （38 cases） of the patients （42 cases） whose naked vision was 0.8/0.33 04） at 6 months after CK, and only 10% （4 cases） of the patients were measured naked vision 0.8/0.33 04） before surgery. The near stereopsis was decreased but not less than 100＂ at 1 month after CK, near stereopsis was improved at 3 months after CK. CK did not effect on far stereopsis （Х^2融 =0.45,Х^2远 = 5.79, P 〉0.05） at 1 month after CK and the value increased alike preoperative at 3 months. Conclusions Distant and near stereopsis are trifling descended earlier period after CK, but there are no statistically signifancence; and with the extension of time stereopsis recover gradually. Near stereopsis recover rapidly and surpass preoperation.
