三维适形放射治疗70岁以上老年人食管癌126例临床分析

摘　　要：目的:回顾性分析70岁以上老年人食管癌三维适形放疗疗效。方法:126例首程治疗的70岁以上老年食管癌患者接受三维适形放疗,113例行常规分割放疗,放疗剂量95%PTV 59.4-64Gy/30-33次。13例前半程行传统常规放疗,处方剂量30-40Gy,分15-20次,后半程行3DCRT加速照射,3.0Gy次/,1次/天,5次/周,总量60-70Gy。结果:126例患者均顺利完成治疗,1、3、5年局部控制率分别为71.4%、47.6%、25.4%;1、3、5年生存率分别为63.5%、32.5%、15.9%。急性放射反应主要是急性放射性食管炎,多为Ⅰ-Ⅱ级。结论:三维适形放射治疗70岁以上老年人食管癌效果满意,不良反应小。Objective：To analyze retrospectively treatment results of three dimensional conformal radiotherapy（3DCRT） on elderly patients with esophageal carcinoma.Methods：From June 2002 to December 2008,126 untreated elderly patients with esophageal carcinoma received 3DCRT,113 patients received conventional fractionated radiation in 1.8-2.0Gy/per fraction,1 fraction/per day,5 fractions/ per week to a total dose of 59.4-64Gy for 95% of PTV,13 patients first received conventional radiotherapy in 2.0Gy/per fraction,1 fraction/per day,5 fractions/ per week to the dose of 30-40Gy in 3-4 weeks,and then received accelerated radiation by 3DCRT in 3.0Gy/per fraction,1 fraction/per day,5 fractions/ per week to the total dose of 60-70Gy.Results：All patients completed the radiation treatment.The 1-,3-,and 5-year local control rates were 71.4%,47.6%,25.4%;The overall 1-,3-,and 5-year survival rates were 63.5%,32.5%,15.9%.Acute radiation esophagitis was the most common radiotherapy but acceptable.Conclusion：Three dimensional conformal（3DCRT）is effective and safe for most of elderly patients with esophageal carcinoma.
