严重外伤性泪囊炎的手术时机与方法及远期效果观察

摘　　要：目的探讨严重外伤引发泪囊炎的治疗时机、方法和远期效果。方法对79例(82眼)因外伤所致慢性泪囊炎,根据不同外伤程度采取相应措施,实施改良式鼻腔泪囊吻合术或全泪道成形术,并进行2a以上的远期效果观察。结果治愈73眼,治愈率为89.02%,其中5眼在术后出现溢泪有黏液性分泌物,行泪道激光后留置插管,2个月后治愈;有效6眼,占7.32%;无效3眼,占3.66%。结论外伤性泪囊炎常伴有眼眶或鼻骨骨折,导致局部解剖结构紊乱,泪道正常结构严重破坏;掌握骨折的特点,选择适当手术方式,提高手术技巧是手术成功的关键。Objective To find the best therapeutic effect by investigating the treatment timing and methods of severe traumatic dacryocystitis.Method For 79 cases（82 eyes） caused by chronic post-traumatic dacryocystitis,to take corresponding measures to implement the modified rhinodacryocystostomy or full dacryocystoplasty depending on the degree of trauma,and conduct long-term effects observed.Results To follow-up observation of two years or more,73 cured,the cure rate of 89.02%,of which 5 eyes in the epiphora occurred after a mucus secretions,cured after 2 months by indwelling catheter after laser lacrimal ducts;effective 6 eyes,efficiency of 7.32% ;invalid 3 eyes,non-efficiency of 3.66%.Conclusion Dacryocystitis often accompanied by traumatic fracture of the orbit or nose,resulting in localized anatomical structural disorder and serious damage to normal structure of the lacrimal ducts,grasping fracture characteristics and choosing an appropriate way of operation,improving surgical skills are key to successful operation.
