摘 要:目的比较屈光参差性单眼弱视儿童患者双眼间的位置辨别功能差异,并探讨潜在的原因。方法前瞻性研究2006年6月至2009年12月在广西视光中心接受视知觉学习的屈光参差性单眼弱视患者80例,其中男43例,女37例,年龄5~15岁。弱视眼的最佳矫正视力为0.12~0.80,对侧健眼的最佳矫正视力为1.0~1.2。所有研究对象按照视力分为弱视眼组和对侧健眼组。一种加入了位置噪声的位置辨别任务被分别用于2组对象的位置辨别检测。该任务需要受试者判断3对线段中未对齐的是哪一对线段(每对线段由2组构成,每组各含8个Gabor斑)。使用SPSS13.0统计软件对2组的最佳矫正视力和位置辨别功能分别进行对比分析。结果弱视眼组与对侧健眼组的最佳矫正视力分别为(0.351±0.156)LogMAR单位、(-0.025±0.044)LogMAR单位,2组间视力差异有显著统计学意义(t=-24.065,P=0.000)。弱视眼组的位置辨别功能的中位数为3级,百分位数P25、P75分别为3级和4级。对侧健眼组的位置辨别功能的中位数亦为3级,百分位数P25、P75亦分别为3级和4级,2组间的差异无统计学意义(MH=-1.481,P>0.05)。结论屈光参差性单眼弱视患者双眼间的位置辨别功能受损程度相似,这提示"健眼"也存在视觉信息加工方面的异常。Objective To compare the differences of position discrimination between the amblyopic eyes and the fellow normal eyes in unilateral amblyopic children with anisometropia,and investigate its underlying mechanisms.Methods From June 2006 to December 2009,80 unilateral amblyopic children with anisometropia were prospectively studied,aged from 5 years old to 15 years old,male in 43 cases and female in 37 cases,undergoing perceptual learning in vision science and Guangxi optometry center.And the best corrected visual acuity(BCVA)ranged from 0.12-0.80 in amblyopic eyes,and from 1.0-1.2 in the fellow normal eyes,whom were divided into amblyopic eye group and fellow normal eye group according to BCVA.A position discrimination task in the presence of positional noise was used to compare the position acuity of two groups.The task was to judge which of 3 pairings in two groups with 8 Gabor patches was misaligned.BCVA and position discrimination of the two groups were compared and analyzed separately by SPSS 13.0 software.Results BCVA in the amblyopic and the fellow normal eyes group were(0.351±0.156)LogMAR unit and(-0.025±0.044)LogMAR unit,respectively;Statistic differences were showed in BCVA between the two groups by paired t test(t=-24.065,P=0.000).The median of position discrimination in amblyopic eye group was 3 levels,while Percentile 25(P25)and Percentile 75(P75)were 3 levels and 4 levels,respectively,which were the same as the fellow normal eye group,but there was no statistical difference between the two groups by Marginal Homogeneity test(MH=-1.481,P0.05).Conclusions Position discriminations in both the amblyopic and fellow normal eyes have been defected to the same extent,which suggests that the fellow normal eye may be abnormal in visual processing.
