小瞳孔下囊膜染色双手连续环形撕囊法超声乳化手术疗效观察

摘　　要：目的探讨小瞳孔白内障囊膜染色双手连续环形撕囊法超声乳化手术的疗效。方法对31例(31眼)小瞳孔白内障患者,行囊膜染色后,以双手法一边拉开虹膜、瞳孔,一边连续环形撕囊,再行超声乳化人工晶状体植入术。术后5d内,每天查视力,观察前房反应、瞳孔状态及人工晶状体状态。结果术后除3眼因反应较重、视力恢复差外,其余28眼均获得了满意视力,术后视力范围0.15～0.80。3眼术后虹膜反应较重,前房闪辉(++);5眼出现轻度虹膜反应,前房闪辉(+);其余23眼仅部分有轻微前房反应。2眼瞳孔膜闭患者术后瞳孔居中,直径约2.5mm,余29眼患者术后瞳孔位置及大小均与术前相当。结论掌握好手术技巧,小瞳孔白内障囊膜染色双手连续环形撕囊法超声乳化手术同样能取得良好的临床效果。Objective To investigate the curative efficacy of microcoria cataract capsular staining and continuous curvilinear capsulorhexis with two-handed technique in phacoemulsification.Methods After capsular staining,a total of 31 microcoria cataract patients（31 eyes）were treated with pulling iris and pupil by two hands,continuous curvilinear capsulorhexis,and then performed phacoemulsification intraocular lens implantation.Within postoperative 5 days,visual acuity was checked everyday,and reaction of anterior chamber,pupil state and intraocular lens condition were observed.Results Except for 3 eyes with severe reaction and poor recovery of visual acuity,the other 28 eyes achieved satisfactory visual acuity postoperatively.Postoperative visual acuity was from 0.15 to 0.8.There were 3 eyes with severe postoperative iris reaction and with anterior chamber flare（＋＋）.Five eyes were with slight iris reaction and with anterior chamber flare（＋）.Parts of the other 23 eyes were with slight reaction of anterior chamber.Pupils of 2 patients,who were with occlusion of pupil,were in the central place postoperatively,and the diameter was about 2.5 mm.Locations and sizes of pupils of 29 eyes at post-operation were equaled to those at pre-operation.Conclusion With excellent operative skills,microcoria cataract capsular staining and continuous curvilinear capsulorhexis with two-handed technique in phacoemulsification can achieve satisfactory clinical efficacy.
