儿童与成人急性B淋巴细胞白血病的免疫分型特点

摘　　要：目的探讨儿童与成人急性B淋巴细胞白血病免疫分型的特点。方法分别选取儿童与成人急性B淋巴细胞白血病患者各100例,采用流式细胞术进行白血病免疫分型检测。结果 1)表达率最高的淋系抗原为CD19(儿童和成人均为100.0%)和CD79a(儿童为93.0%;成人为90.0%);儿童和成人组中,伴髓系抗原表达率分别为32.0%和42.0%,均以CD33表达率最高,CD13次之,CD117最低。2)大多数抗原的表达率在两组间并无差别,但下列除外:Pro-B-ALL的CD22(儿童>成人)和CD38(儿童<成人)、C-B-ALL的CD20和CD22(儿童>成人)及CD34(儿童<成人)、Pre-B-ALL的CD34和CD38(儿童<成人)。结论 1)CD19和CD79a为淋系最敏感的抗原,伴髓系抗原的表达率在两组间并无差别。2)成人组CD34和CD38的高表达可能是预后不良因素,儿童组CD20和CD22的高表达可能有利于预后。Objective To investigate the characteristics of immunophenotype in B cell acute lymphoblastic leukemia among childhood and adult. Methods Immunophenotyping was performed by flow cytometry in 100 B cell acute lymphoblastic leukemia of children and adults respectively. Results 1）The highest incidence of lymphoid associated antigens were CD19（ both 100% among children and adults groups ） and CD79a（ 93.0% in children; 90.0% in adults）. Th＇e incidence of ALL with myeloid antigen expression（My＋ALI.） was 32% in children group and 42% in adults group. Among them,CD33 was the highest one,followed by CD13, CDll7 in turn. 2）No significant difference of positive rate of lymphoid associated antigens was found between children and adults group,except the followings：CD22（children 〉 adults） and CD38（children 〈adults） in ProB ALL groups; CD20,CD22（children〉 adults）and CD34（children〈adults） in C,B ALL; CD34 and CD38（children〈adnhs） in PreB ALL. Conclusion 1）CD19 and CD79a were the most sensitive lymphoid associated antigens. No difference of ALL incidence with myeloid antigen expression existed between children and adults. 2）Higher expression rate of CD34 and CD38 in adults might be considered as a poor prognostic factor, while higher positive incidence of CD20 and CD22 in children might be favorable for the recovery.
