幽门螺杆菌感染对功能性消化不良患者胃酸分泌的影响

摘　　要：目的探讨幽门螺杆菌（Ｈｐ）感染对功能性消化不良（ＦＤ）患者的胃酸和胃泌素分泌的影响。方法５４例符合ＦＤ诊断标准的患者，其中Ｈｐ阳性２３例，Ｈｐ阴性３１例，所有患者都进行空腹血清胃泌素、基础胃酸排出量（ＢＡＯ）、五肽胃泌素刺激后的最大胃酸排出量（ＭＡＯ）和高峰胃酸排出量（ＰＡＯ）的测定。对照组Ｈｐ阳性的十二指肠溃疡病患者５５例。结果在ＦＤ患者中，Ｈｐ阳性者和Ｈｐ阴性者比较，空腹血清胃泌素（１０３１±３３７ｐｇ／ｍｌｖｓ１１３．３±３４１ｐｇ／ｍｌ）和ＢＡＯ（５２１±３８６ｍｍｏｌ／ｈｖｓ４．８０±６０８ｍｍｏｌ／ｈ）无明显差异（Ｐ＞００５）；而Ｈｐ阳性者的ＭＡＯ和ＰＡＯ（分别为１６３±９３０ｍｍｏｌ／ｈ和２３６±１４２ｍｍｏｌ／ｈ）与Ｈｐ阴性者（分别为１０１±８８８ｍｍｏｌ／ｈ和１４２±１１３ｍｍｏｌ／ｈ）比较，差异有显著性（Ｐ＜００５）。Ｈｐ阳性的ＦＤ患者，其ＢＡＯ、ＭＡＯ、ＰＡＯ及空腹血清胃泌素与Ｈｐ阳性的十二指肠溃疡病患者（后者分别为１０４±０８１ｍｍｏｌ／ｈ、２４２±１０８ｍｍｏｌ／ｈ、３１．２±１３．１ｍｍｏｌ／ｈ和１４８５±１３．１ｐｇ／ｍｌ）比较，差异有?Aim This study was undertaken to determine whether Helicobacter pylori (Hp) positive patients with functional dyspepsia (FD) have an acid secretion disturbance. Methods: Fasting serum gastrin basal and pentagastrin stimulated acid output were examined in 23 Hp positive FD patients and the results were compared with those of 31 Hp negative FD patients and 55 Hp positive DU patients. Results: Compared with the Hp negative FD patients,fasting serum gastrin (103.133.7 pg/ml vs 113.334. 1 pg/ml) and basal acid output (5.213.86 vs 4.806.08 mmol/h) were no difference (P>0.05) for the Hp positive FD patients;pentagastrin stimulated acid secretion (MAO and PAO) was higher in Hp positive FD patients (16.39.30 mmol/h and 23.614.2 mmol/h)than Hp negative FD patients (10.18.88 mmol/h and 14.211.3 mmol/h,P<0.05).The Hp positive FD patients,however,had lower serum gastrin and acid output than Hp positive DU patients (148.513.1 pg/ml210.40.81 mmol/h2421.08 mmol/h and 31.213.1 mmol/h,respectively,P<0.05). Conclusions: The pentagastrin stimulated gastric acid output is higher in Hp positive FD patients than Hp negative FD patients,but lower than Hp positive DU patients.It is suggested that the parietal cells in Hp positive FD patients were more sensitive to pentagastrin stimulation than Hp negative FD patients Hp infection seems likely to play a role in the gastric acid secretion of FD patients.
