摘  要:目的探讨肌电诱发神经肌肉电刺激改善脑卒中患者偏瘫下肢运动功能的临床疗效。方法选择60例脑卒中偏瘫患者,随机分3组,第1组为常规康复治疗组;第2组为在常规康复治疗的基础上加神经肌肉电刺激;第3组在常规康复治疗基础上加用肌电诱发的神经肌肉电刺激。测定上述患者肌肉静态及收缩时肌电值,应用简式Fugl-Meyer功能评价表评价患者下肢运动功能,在疗程开始与结束时分别予以评价和比较。结果第3组患者治疗前后的肌肉静态及收缩时肌电值、应用Fugl-Meyer评价表进行评分的分值均有显著性提高,在改善下肢运动功能上明显优于单纯康复训练和在康复训练的基础上加用神经肌肉电刺激治疗(P<0.05)。结论肌电诱发的神经肌肉电刺激疗法是一种有效的临床康复治疗方法,可以用于偏瘫患者足下垂治疗,其疗效优于单纯常规治疗方法和神经肌肉电刺激疗法。Objective To study the efficacy of electromyography-triggered neuromuscular stimulation in improving the lower extremity motor functional of stroke survivors. Methods Sixty stroke survivors who lost the ability of ankle dorsoflexion were randomly assigned to 3 groups. The first group: 20 patients had received routine rehabilitation training; The second group: 20 patients had received an extra therapy--neuromuscular electrical stimulation; The third group: 20 patients had received an extra therapy—electromyography-triggered neuromuscular stimulation. The observed value of EMG and Fugl-Meyer functional assessment were recorded before and after treatment. Results The observed value of EMG and Fugl-Meyer functional assessment were statistically improved in the third group. Conclusion This study indicate that electromyography-triggered neuromuscular stimulation is an efficient technique in treating stroke-induced hemiplegia which can enhances the motor function of hemiplegia, and is better than neuromuscular electrical stimulation.
