肝脏少见病例超声造影表现

摘　　要：目的分析肝脏少见病例超声造影表现。方法对常规超声检查不能确诊的肝脏局灶性病变患者,进行超声造影检查并获得明确病理检查结果的患者129例,对其中7例肝脏少见病例超声造影表现进行回顾性分析。结果 2例肝胆管细胞癌中1例表现为动脉相病灶环状增强,增强部分呈快进快出;另1例为瘤体整体呈不规则增强,增强呈快进快出。1例肝紫癜表现为动脉相病灶部分增强,增强呈快进快出。1例肝血管上皮样肿瘤表现为动脉相病灶整体增强,增强呈快进慢出。1例肝副神经节瘤表现为动脉相病灶整体增强,增强呈快进慢出。1例肝炎性假瘤超声造影各时相均未见增强。1例肝外伤后血肿超声造影各时相均未见增强,病灶区大小测量较二维超声时增加。结论肝少见病例病理类型不同其超声造影表现不同;部分病种有多种组织学分型,导致同一类型的肿瘤造影表现不同。Objective To investigate the enhancement appearance of special focal liver lesions on contrast-enhanced ultrasonography（CEUS）.Methods 129 cases of focal liver lesions not diagnosed by common ultrasound were examined by contrast-enhanced ultrasound,and the results were confirmed by pathology.Finally 7 special cases were retrospectively analyzed.Results In 2 cases of intrahepatic cholangiocellular carcinoma,one showed peripheral hyper-enhancement quickly in early arterial phase and disappeared quickly in portal vein phase and late phase,and one exhibited homogeneous hype-enhancement within the lesion in the arterial phase.One case of peliosis hepatics showed part of the lesion was enhanced with a pattern of ＂quickly-in and quickly-out＂.In one case of perivascular epithelioid cell tumor of liver,the lesion was enhanced quickly in early artery phase and disappeared slowly in portal vein phase and late phase.In one case of paraganglioma of liver the lesion was enhanced quickly in early artery phase and disappeared slowly in portal vein phase and late phase.One case of hepatic inflammatory pseudotumor and one case of hepatic trauma had no enhancement in all phases,and the area of the trauma was bigger than that measured by two-dimensional ultrasound.Conclusion Different liver tumors were characterized by different enhancement patterns on the CEUS.The different components of tumor cells and mesenchymal tissue in the same type tumor can show different enhancement patterns.
