事件相关电位对帕金森病患者认知功能损害的评价

摘　　要：目的通过事件相关电位(ERP)观察非痴呆帕金森病(PD)患者识别、区分熟悉和不熟悉人名的认知能力有无损害,以及ERP变化与认知功能损害程度的相关性。方法采用EMS32导脑电地形图机及其处理系统,以熟悉和不熟悉的人名作为诱发ERP的任务,观察32例PD病患者事件相关电位P300波群的变化。结果 (1)PD患者组熟悉人名和不熟悉人名诱发的P3波幅之差较对照组明显缩小(P<0.05);(2)PD患者无论熟悉人名与不熟悉人名诱发的N2、P3潜伏期与MMSE评分呈显著负相关性(P<0.01),MMSE评分24分～26分PD患者组与正常对照相比,N2、P3潜伏期显著延长(熟悉:P<0.01;P<0.01;不熟悉:P<0.01;P<0.01),而P3波幅与正常对照之间则无显著差异(P>0.05)。结论非痴呆PD患者识别熟悉人名的认知能力下降,事件相关电位潜伏期延长可反映PD患者轻度认知功能损害,并与其损害程度相关。Objective To evaluate whether patients with nondemented Parkinson disease（PD） suffer from cognitive impairment of identifying and distinguishing familiar and unfamiliar names with event-related potentials（ERP）,and correlation between changes of ERP and the degree of cognitive impairment was also investigated.Methods The changes of event-related potential P300 induced by familiar and unfamiliar names in 32 patients with nondemented PD were evaluated using EMS 32 lead brain electrical activity map and its processing system.Results（1）The differences between P3 wave amplitudes induced by familiar and unfamiliar names significantly reduced in patients with nondemented PD compared with controls（P〈0.05）;（2） The latencies of N2,P3 induced by either familiar or unfamiliar names had a significant negative correlation with MMSE scores（P〈0.01）,the latencies of N2,P3 was markedly longer in PD patients with MMSE scores of 24-26 than controls（familiar：P〈0.01;P〈0.01;unfamiliar：P〈0.01;P〈0.01）,but the wave amplitude of P3 showed no significant difference between the two groups（P〈0.05）.Conclusion The cognitive function of identifying familiar names is impaired in patients with nondemented PD,and prolonged latencies ERP can reflect the mild cognitive impairment,which correlated with its impaired degree.
