P53联合CA125半衰期检测在评价卵巢癌预后中的作用

摘　　要：目的探讨P53联合CA125半衰期检测与卵巢癌患者预后的关系。方法应用免疫组化方法检测35例卵巢癌组织和22例正常卵巢组织中P5，的表达情况，在术前、化疗及随访过程中采集静脉血用放射免疫法测定CA125浓度。结果P53在卵巢癌组织中的阳性表达率为77．1％，而正常卵巢组织中无表达，两者比较有统计学意义（P〈0．01）；P53表达强度与CA125半衰期之间有相关性，即CA125半衰期〉3周者与CA125，半衰期≤3周者比较，前者P53表达强度高于后者，两者比较有统计学意义（P〈0．05）；2年内复发者P53高表达率高于2年后复发者（P〈0．05）；CA125半衰期〉3周者2年复发率明显高于CA125半衰期≤3周者（P〈0．05）；分级评价CA125半衰期及P53的表达率，Ⅲ-Ⅳ期卵巢癌患者的2年及3年生存率之间差异均有显著性（P〈0．05）。结论P53与CA125半衰期的联合检测可作为判断卵巢癌的预后及指导临床治疗的指标。Objective To study the association of expression of P53 and half life time of CA125 with outcome in ovarian cancer patients. Methods Immunohistochemistry method was adopted to examine the expression of P53 in 35 cases with ovarian cancer and 22 cases with normal ovary. The dynamic serum levels of CA125 before and after operation were measured by radioimmunoassay method weekly. Results The positive rate of P53 in Ovarian cancer was 77.1%, which was significantly higher than that in normal ovarian tissue（P 〈0. 01 ）. The expressional intensity of P53 has correlation with the haft life time of CA125. It was higher in cases whose half life period of CA125 were over three weeks than in those less than or equal to three weeks（ P 〈 0. 05 ）. The high expressing rates of P53 in patients with recurrence in two years were higher than those after two years （ P 〈 0. 05 ） ; The recurrence rate in two years whose half period of CA125 were over three weeks was significantly higher than those less than or equal to three weeks （ P 〈 0. 05 ）. Evaluating by grade in half - life time of CA125 and the expression rate of P53 in Ⅲ-Ⅳ ovarian cancer for 2 years and 3 year survival rates was significant differences （ P 〈 0.05 ）. Conclusion The expressional intensity of P53 in combination with the half life time of CA125 may be used as a prognostic marker in ovarian carcinoma and is helpful to guide treatment.
