活血化瘀法治疗溃疡性结肠炎的临床研究

摘　　要：目的:研究活血化瘀法治疗溃疡性结肠炎的有效性和安全性。方法:选择溃疡性结肠炎患者60例进行前瞻性、随机、西药治疗平行对照的临床试验。试验组和对照组各30例,试验组口服活血化瘀中药汤剂,对照组口服柳氮磺胺吡啶片。疗程为8周。于治疗前后观察并记录腹痛、脓血便等症状以及结肠镜下所见。停药6个月内对显效和有效的病人保持随访。结果:临床症状疗效:试验组在控制脓血便症方面显著优于对照组(P<0.05);黏膜病变疗效:试验组与对照组总有效率分别为90.0%和76.7%,两组间比较差异无统计学意义(P>0.05);溃疡性结肠炎综合疗效:试验组与对照组总有效率分别为90.0%和76.7%。远期疗效:试验组有7例复发或加重,对照组有14例复发或加重,试验组远期疗效优于对照组(P<0.05)。结论:活血化瘀法治疗溃疡性结肠炎有效,尤其在控制脓血便方面,并且具有良好远期疗效和安全性。Objective：To study the effectivity and security of the method of activating blood circulation to dissipate blood stasis on ulcerative colitis.Methods：A prospective,randomized controlled trial was conducted.60 patients were divided into 2 groups randomly.Trial group（n=30）took the traditional Chinese medicine decotion having the effect of activating blood circulation to dissipate blood stasis,control group（n=30）took Sulfasalazine tablet（SASP）.The therapeutic course for both groups was 8 weeks.Items should be observed or detected as follows at pretherapy and post-treatment：（1）the score of symptoms;（2）the finding of colonoscope.Followed up patients that of complete remission and excellence after drug withdrawal 6 months.Results：Improving of symptom：There was statistical significance between two group on scores of bloody purulent stool after treatment,trial group was better than control group.Therapeutic effect of affected mucosa：the total effective rate Therapeutic effect of synthetic：analysis showed that they were 81.08% and 80.56% respectively（P0.05）.Protective efficacy：There were 7cases and 14cases recidivating or aggravation in trial group and control group respectively.There was significant difference on recurrence rates between two group（P0.05）.Conclusion：treating UC with the method of activating blood circulation to dissipate blood stasis is effective and safe.Its prostective efficacy is better than SASP.
