螺旋CT多期增强扫描在肾癌诊断中的应用价值

摘　　要：目的探讨螺旋CT多期增强扫描在肾癌诊断中的应用价值。方法对我院30例经螺旋CT检查,并经手术病理证实的肾癌病例进行分析。结果肾癌螺旋CT平扫22例表现为低混杂密度,其中8例见低密度坏死区和5例有斑点、弧形钙化及3例微量脂肪变;5例表现为等密度;3例呈较低密度囊状影。肿瘤边界模糊不清19例,清晰而不锐利8例,边缘相对光整3例。三期增强扫描于动脉期肿瘤实质中度—明显强化22例,强化程度CT值大于40HU,其中8例低密度坏死区无强化;3例壁结节中度—明显强化;轻度强化5例。实质期肿瘤病灶密度迅速降低,排泌期病灶均低于实质期密度。结论利用螺旋CT三期增强扫描观察病灶的密度、边缘及强化程度的特征性表现,对肾癌的术前诊断具有较高的准确性。Objective Inquire into spiral CT more expecting to strengthen to scan to diagnose in the kidney cancer in of application value. Method Is 30 to my hospital through spiral CT check,and the kidney cancer case confirmed by surgical operation pathology carry on analysis.Result Kidney cancer the spiral CT Be even to sweep 22 performances as low congestion densities, 8 among them see the bad dead area and 5 of low density having spot, arc-shaped calcify and 3 little fats to change;5 performances are to wait density;3 presents a lower density bag-like in shape shadow. The tumor boundary is 19 unclear, 8 clear but not sharp-edged, the edge is only opposite the whole 3.3 strengthens a scanning to expect tumor at the artery substantial mid-degree-obviously enhance 22, enhance degree CT to be worth is bigger than a 40 Hus, 8 among those examples low density the bad dead area didn＇t enhance;3 wall knot the stanza is mid-degree-obviously enhance;The light degree enhances 5.Substantial period tumor focus the density quickly lower and line up Bi to expect a focus is aller low than a substantial period density.Conclusion Make use of spiral CT three densities, edges that strengthen a scanning observation focus and enhance the characteristics of degree performance, have higher accuracy to the before the Shu diagnosing of the kidney cancer.
