摘  要:目的观察TNF-a对慢性阻塞性肺疾病(COPD)营养不良模型大鼠呼吸肌萎缩的影响以及吲哚美辛(IND)对其的抑制作用。方法采用反复熏香烟加气道内滴猪胰弹性蛋白酶方法建立COPD大鼠模型,把COPD营养不良大鼠随机分为A、B、C、D 4组。A组、对照组和COPD营养正常组每天予等量生理盐水灌胃,B、C、D组每天予不同剂量IND灌胃(B组:IND 0.5 mg/kg.d,C组:IND 1 mg/kg.d,D组:IND 2 mg/kg.d)。检测各组干预前后体重、血清中TNF-α的浓度以及大鼠呼吸肌蛋白质分解代谢率。结果 IND干预后,干预组B、C、D血清中TNF-a含量较干预前明显降低(P<0.01),比干预A组显著性的降低(P<0.01),同时大鼠呼吸肌蛋白质分解代谢率也下降、体重回升,干预组B、C、D大鼠体重增长速度高于A组,在C组达到显著性(P<0.05)。结论 TNF-α参与了COPD营养不良、骨骼肌萎缩的发生,吲哚美辛可以降低COPD营养不良大鼠血清中TNF-α的水平以及骨骼肌蛋白分解代谢率,从而部分改善COPD营养不良大鼠的骨骼肌萎缩。【Objective】To observe the influence of tumor necrosis factor-alpha(TNF-a) on respiratory muscle atrophy in COPD malnutrition rats and the inhibition of indomethacin(IND) on it.【Methods】Model group was performed with intratracheal instillation of porcine pancreatic elastase and exposed to cigarette smoke to establish COPD models.The malnutrition group rats were divided by randomized block design into four groups(group A,B,C,D).Isotonic physiologic saline was administered to group A,the control group and the non-malnutrition COPD group,and different doses of oral IND were administered to groups B,C,and D daily(group B:IND 0.5 mg/kg.d,C: IND 1 mg/kg.d,D: IND 2 mg/kg.d).The changes of body weight,the concentrations of TNF-α in the serum,respiratory muscle homogenate,as well as respiratory muscle protein catabolism rates were detected in each group before and after the intervention.【Results】The TNF-α concentrations of the serum of rats of the intervention groups B,C and D after the intervention were significantly lower than before the intervention(P 0.01),and were significantly lower than the group A(P 0.01).At the same time,respiratory muscle protein catabolism rates were decline and body weight was recovery.The body weight growth rate of the intervention groups B,C and D rats were higher than the group A,the intervention group C achieved significant(P 0.05).【Conclusion】TNF-α is involved in the occurrence of COPD malnutrition and respiratory muscle amyotrophy.IND can reduce the TNF-α levels in the serum and the catabolic rates of the respiratory muscle proteins in malnutrition COPD rats,which can improve partly the the nutritional status of malnutrition COPD rats and reverse the respiratory muscle atrophy.
