摘 要:目的:探索空肠弯曲菌(CJ) 外膜上神经节苷脂样结构是否为诱导周围神经传导阻滞的关键抗原结构,为CJ诱发吉兰-巴雷综合征(Guillain-Barré,syndrome,GBS)的分子模拟理论提供证据.方法:利用CJ亲代株(含神经节苷脂样结构)及waaF变异株(缺失神经节苷脂样结构)作为抗原,进行对照研究.将26只日本大耳兔随机分为CJ亲代株组(10只)、waaF变异株组(10只)及对照组(6只),分别用亲代株CJ、waaF变异株及免疫佐剂,对三组动物进行全身免疫.于免疫前及免疫后第4、8周进行神经电生理检查,观察坐骨神经复合肌肉动作电位(CMAP)、坐骨神经运动传导速度(MCV)、F波出现率的变化.结果:神经电生理检查显示:免疫后第4周,亲代株组动物坐骨神经CMAP波幅较免疫前明显降低,差异显著 (P〈 0.05);第8周时CMAP波幅较第4周进一步下降;变异株组动物坐骨神经CMAP波幅在第4周及第8周时与免疫前比较无明显差异;三组动物坐骨神经MCV在免疫前、免疫后第4及第8周比较差异均无显著意义(P〉 0.05);免疫后亲代株组动物坐骨神经F波出现率减少,而变异株组无明显改变.结论: CJ外膜上神经节苷脂结构可诱导动物轴索型周围神经传导阻滞,支持CJ感染相关轴索型GBS的分子模拟发病理论.Objective:To determine whether ganglioside-like epitopes in Campylobacter jejuni (CJ) are critical antigen to induce conduction block of peripheral nerves and find an experimental evidence for the presumption of molecular mimicry on the pathogenesis of Guillain Barr4 syndrome (GBS) after CJ antecedent infection. Methods: Comparison study was made by using the CJ parent strain (with ganglioside like epitopes) and its direct waaF gene knockout mutant (waaF mutant without ganglioside-like epitopes). Twenty-six Japanese white rabbits were randomly divided into three groups:the parental strain group (CJ group,n=10),the waaF mutant group (n=10) and the control group(n=6), and immunized with Freund's adjuvant (FA) adding parental strains of CJ HB9313, waaF mutants and saline respectively. Electrophysiological examinations were carried out for the sciatic nerves of the 3 groups in the 4th and 8 th weeks after immunization including amplitude of compound muscle action potentials (CMAPs), motor nerve conduction velocity (MCV), frequency of F wave persistence. Results: The amplitude of CMAPs CJ group, in the 4th week after immunization was significantly lower than those before immunization (P〉 0.05), in the 8th week was lower than that in the 4th week,but the difference was not significant (P〈 0.05). The amplitude of CMAPs in waa F group,in the 4th and 8th weeks after immunization had no difference with those before immunization. The difference between the waa F group and the controis was also not significant. The MCV of sciatic nerves in the 3 groups showed no significant difference(P〉 0.05). 50 % of rats immunized with parental strains in the 4th week showed F wave persistence decreased,in the 8th week to 66.7%. However, waaF mutants and the control showed the normal F wave persistence. Conclusion: The ganglioside-like epitopes of CJ is critical antigen to induce axonal conduction block of peripheral nerves, so as to provide an evidence for the molecula
