摘 要:目的研究乳腺导管癌中MMP-2、MMP-9与ColⅣ的表达及其相关性,并探讨其与临床病理参数之间的关系。方法应用免疫组化双染法检测60例乳腺导管癌中MMP-2、MMP-9和ColⅣ的表达,并进行相关性分析,同时分析MMP-2、MMP-9与乳腺导管癌患者的年龄、肿瘤大小、淋巴结转移及肿瘤ER、PR、E-cad、C-erbB-2的关系。结果 MMP-2、MMP-9在乳腺导管癌中的表达显著高于乳腺正常组织(P<0.05),而ColⅣ在乳腺癌组织中的表达低于乳腺正常组织(P<0.05)。MMP-2、MMP-9在靠近基膜或者于将要突破基膜处的癌细胞中表达较强。MMP-2、MMP-9在不同年龄组、PR、ER、E-cad分组间的表达无显著差异(P>0.05);但在肿瘤直径>2 cm、有淋巴结转移及C-erbB-2阳性组的表达显著高于相应的对照组(P<0.05)。ColⅣ的表达与MMP-2的表达呈负相关(r=-0.506,P=0.000),亦与MMP-9的表达呈负相关(r=-0.501,P=0.000)。结论 MMP-2、MMP-9对ColⅣ的降解可能在乳腺导管癌的局部侵袭和远处转移过程中发挥重要作用。MMP-2、MMP-9和ColⅣ的表达,可能对判断乳腺癌的恶性程度和浸润转移潜能以及评估预后有参考价值。Objective To research the expression and correlation of MMP-2, MMP-9 and Col IV, investi- gate their relationship to the clinical parameters. Methods hnmunohistochemistry double staining was used to detect the expression of MMP-2, MMP-9 and Col IV on 60 cases of breast duetal carcinomas. Moreover, the relations between MMP-2, MMP-9 and breast ductal carcinomas patient age, tumor size, histological type, lymph node metastasis, tumor ER, PR, E-cad and C-erbB-2 were analyzed. Results The expression of MMP-2, MMP-9 in breast duetal carcinomas was significantly higher than in normal breast tissues (P 〈 0.05 ) , but the expression of Col IV in breast ductal carcinomas was lower than in normal breast tissues (P 〈 0.05 ). MMP-2, MMP-9 was expressed more strongly in the cancer cells, which approach the basement membrane or will have to break through the basement membrane. The expression of MMP-2, MMP-9 between different age groups, ER, PR and E-cad was no significant difference (P 〉 0.05) ; however, the expression in tumor diameter 〉 2 era, with lymph node metastasis and C-erbB-2 positive group was significantly higher than the eorreponding control group (P 〈0.05 ). The expression of MMP-2 and Col IV was negatively correlated( r = -0. 506, P = 0. 000) , also the expression of MMP-9 was a negative correlation(r = - 0. 501, P = 0. 000). Conclusions The degradation of Col IV by MMP-2 and MMP-9 may be played an important role in local invasive and distant metastasis of breast ductal carcinomas. The expression of MMP-2, MMP-9 and Col IV may have reference value in determining the degree of malignaney and the invasive and metastatic potential of breast cancer, and evaluating prognosis.
