摘  要:目的分析实时荧光定量PCR检测血浆中人乳头瘤病毒(human papilloma virus,HPV)16、18的可行性,并探讨其对宫颈癌诊断和预后评估的价值。方法收集2008年6月至2009年5月确诊的宫颈癌患者140例,癌前病变10例,以及正常对照者35例。采用改良通用引物PGMY和PCR-反向点杂交法对宫颈活检组织进行HPV基因扩增与分型。应用real-time PCR方法检测血浆HPV16、18 DNA水平,观察HPV DNA的感染率和病毒含量,分析其与宫颈癌临床分期的相关性。结果在宫颈组织HPV16、18阳性患者中,血浆HPV感染率分别为57.9%和23.8%。随着宫颈癌临床病期的发展,血浆HPV16 DNA检出率逐渐增加(P<0.001),但病毒含量的变化无统计学意义(P=0.277)。血浆HPV18病毒检出率和病毒拷贝数与宫颈癌临床分期均无相关(P=0.609,P=0.512)。结论血浆HPV DNA定量检测是一种可靠的实验方法,对宫颈癌的诊断和预后监测具有重要价值。Objective To prospectively evaluate the feasibility of detecting plasma human papilloma virus (HPV) type 16 and 18 DNA of patients with cervical cancer using real-time polymerase chain reaction (PCR) and to determine its prognostic importance. Methods 150 women with cervical neoplasia, comprising 10 with CIN, 140 with invasive cervical cancer were collected, and 35 normal persons as control. HPV DNA was detected in cervical tissue using consensus PGMY primers and genotyped using reverse line blot hybridization. HPV 16 and 18 detection and quantitation in plasma was performed using real-time PCR to investigate the prevalence and viral load of HPV DNA and analysis the correlation with clinical disease stage. Results Viremia was detected in 57.9% of HPV 16 positive women and 23.8% of HPV 18 positive women. The prevalence of HPV 16 DNA in plasma increased with advancing disease stage ( P 〈 0. 001 ) , although HPV 16 absolute plasma viral load was not significantly associated with advancing disease stage (P = 0. 277). Neither the rate of detection of HPV 18 plasma viremia nor absolute plasma viral load was associated with advancing clinical disease stage (P = 0. 609,P = 0. 512). Conclusions Plasma HPV DNA determination by real-time PCR is a reliable method and the presence of HPV16 might be a valuable marker for monitoring therapeutic response and disease progression in cervical cancer.
