不同年龄段Ⅲ类骨性畸形垂直向发育性差异的Delaire头影测量初步分析

摘　　要：目的:采用Delaire头影测量分析法比较重庆地区不同年龄段Ⅲ类骨性畸形患者的垂直向发育性差异。方法:重庆地区不同垂直骨面型Ⅲ类骨性畸形患者儿童(9～11岁)30例、成人(19～21岁)44例,采用计算机辅助Delaire头影测量方法对其颅颌面硬组织进行标记测量,SAS9.0行假设检验分析,当方差齐性时取t检验值,方差不齐时取Satterthwaitet检验值。结果:获取了各样本的Delaire头影测量值。发现高角组内(∠2、∠4、(Pts-M)/C2、(M-Cp)/C3(%)、Np-F1)成人与儿童之间角度的差异具有统计学意义(P<0.05);低角组内(∠1、(Pts-Cp)/C2)成人与儿童之间角度的差异具有统计学意义(P<0.05);其他指标尚未见明显差异。结论:随个体生长发育,不同垂直骨面型以及不同年龄阶段的Ⅲ类骨性畸形在颅、面、颌、颈三维方向均具有一定的发育性差异,这种差异可指导临床矫治对于不同情况的Ⅲ类骨性畸形进行针对性的矫治设计,为正畸正颌联合治疗提出更为精确的治疗方案,以利于治疗的长期效果。Objective：To evaluate the vertical direction developmental difference between skeletal Class Ⅲ malocclusions in different age groups in Chongqing by Delaire cephalometric analysis.Methods：30 children patients（age 9~11） and 44 adult patients（age 19~21） of Class Ⅲ malocclusion with different vertical skeletal facial profile were recruited into the research,which were marked and analyzed by computer aided Delaire cephalometric analysis.Hypothesis testing was analyzied by SAS9.0,and independent-samples t-test were used to analyze the data when homogeneity of variance,while Satterthwaite t-test were used to analyze the data when variance arrhythmias.Results：The Delaire cephalometric values of these samples were obtained,the difference of ∠2,∠4,（Pts-M）/C2,（M-Cp）/C3（%）,Np-F1in high-angle group between adult and children were statistically significant（P0.05）;While the difference of ∠1,（Pts-Cp）/C2 in low-angle group between adult and children were statistically significant（P0.05）.However,there were no significant differences in other indicators.Conclusion：With the individual growth and development,the Class Ⅲskeletal deformities with different vertical skeletal facial profile and different age present definite developmental difference in a three-dimensional orientation of the skull,face,jaw,and the neck.And the difference is deserved to guide the orthodontic treatment planning which for the different Class Ⅲ skeletal deformities.It is also beneficial to the exacter therapeutic regimen which combination of orthodontics and straight jaw,and stability for the long-term effect.
