摘 要:目的探讨人参皂甙Rd对脊髓缺血性损伤保护作用的剂量效应关系。方法 40只雄性新西兰大白兔,随机分为5组(n=8),采用肾下主动脉阻断法造成脊髓缺血(20min)。对照组:即单纯缺血再灌注组;保护组:即Rd-5组、Rd-10组、Rd-20组和Rd-40组,分别在缺血前1h从耳缘静脉注射人参皂甙Rd5mg/kg、10mg/kg、20mg/kg、40mg/kg;术后观察神经功能变化并记录再灌注4h、8h、12h、24h和48h神经功能学评分,再灌注48h处死动物后取脊髓(L5~7)标本行病理学观察。结果所有动物均存活,再灌注后48h,Rd-5组神经功能学评分和脊髓前角正常运动神经元计数与对照组相比均无显著性差异(P>0.05);Rd-10组神经功能学评分与对照组相比无显著性差异(P>0.05),但脊髓前角正常运动神经元计数明显高于对照组(P<0.05);Rd-20组、Rd-40组神经功能学评分和脊髓前角正常运动神经元计数均明显高于对照组(P<0.01),但这二组之间无显著性差异(P>0.05)。且每组兔神经功能学评分与其对应脊髓前角正常神经元计数之间有显著相关性(r=0.769.P<0.01)。结论人参皂甙Rd对脊髓缺血性损伤有保护作用,且呈一定的剂量效应关系。Objective To investigate the dose-response relationship between ginsenoside Rd injection and ischemic injury of spinal cord in rabbits.Methods Forty male New Zealand white rabbits were randomly divided into 5 groups (n=8). Spinal cord ischemia was induced for 20 min by infrarenal aortic occlusion under anesthetization with pentobarbital sodium. Control group received no pharmacologic intervention. Group Rd-5,Rd-10,Rd-20 and Rd-40 received intravenous infusion of 5 mg/kg,10 mg/kg,20 mg/kg and 40 mg/kg ginsenoside Rd within 1 h before aortic occlusion,respectively. Neurological function was scored at 4 h,8 h,12 h,24 h,and 48 h after reperfusion. Tissue (L5~7) for histopathologic examination was prepared by hematoxylin-eosin staining at 48 h after reperfusion.ResultsAll animals survived in the experiment. Forth-eight hours after reperfusion,there was no statistic difference in neurological function score and the number of normal neurons of anterior spinal cord in group Rd-5 compared with control group (P0.05); there was statistic difference in normal neurons of anterior spinal cord in group Rd-10 but no difference in neurological function score compared with control group (P0.05); there was significant statistic difference in neurological function score and the number of normal neurons of anterior spinal cord in group Rd-20 and Rd-40 compared with control group (P0.01),but no statistic difference was found between group Rd-20 and Rd-40 (P0.05). There was significant correlation between neurological function score and the number of normal neurons of anterior spinal cord in each group (r=0.769. P0.01). Conclusion Ginsenoside Rd injection can effectively reduce neurological injury induced by spinal cord ischemia and reperfusion in a dose-response manner within a certain dosage range.
