急性胰腺炎腹膜后间隙受累的CT表现与临床严重程度的相关性分析

摘　　要：目的探讨急性胰腺炎(acute pancreatitis,AP)腹膜后间隙受累的CT表现与临床严重程度的相关性。资料与方法回顾性分析76例AP患者的临床和影像学资料,按照亚特兰大AP临床分类标准及AP腹膜后间隙扩散CT表现范围分级评分。分析腹膜后间隙扩散CT表现范围分级评分与临床严重程度分级评分的相关性。结果 76例中,肾旁前间隙、肾周间隙及肾旁后间隙受累发生率分别是49%(37/76)、25%(19/76)及26%(20/76)。轻症AP26例,其中肾旁前间隙受累19例,肾周间隙受累6例,肾旁后间隙受累1例;重症AP50例,其中肾旁前间隙受累18例,肾周间隙受累13例,肾旁后间隙受累19例。腹膜后间隙扩散CT表现范围分级评分与临床严重程度呈显著正相关(r=0.547,P=0.000)。结论 AP腹膜后间隙扩散CT表现范围分级评分与临床严重程度密切相关,影像学检查能够为AP的严重程度及预后提供重要的信息。Objective To discuss the relationship between clinical severity and CT features of acute pancreatitis（AP） involving retroperitoneal space. Materials and Methods CT and clinical data of 76 patients with AP were retrospectively analyzed. According to the Atlanta classification standard,the relationship between clinical severity and CT features of acute pancreatitis（AP） was evaluated according to CT grading of AP involving the retroperitoneal spaces. Results The incidence of anterior pararenal space,perirenal space,posterior pararenal space involvement were 49%（37/76）、25%（19/76）、26%（20/76）,respectively. Among 76 cases,26 cases were mild AP,anterior pararenal space,perirenal space,posterior pararenal space were involved in 19,6 and 1 cases respectively; 50 cases were severe AP,anterior pararenal space,perirenal space,posterior pararenal space were involved in 18,13 and 19 cases respectively. CT grading of AP involving the retroperitoneal spaces had positive correlation with clinical severity（r=0.547,P=0.000）. Conclusion CT grading of AP involving the retroperitoneal spaces closely relates with clinical severity and also provides important imaging information for the clinical severity and the prognosis of AP.
