同期放化疗联合辅助化疗在中晚期子宫颈癌中的应用

摘　　要：目的:观察子宫颈鳞癌患者同期放化疗联合辅助化疗疗效。方法:138例2a～3b期患者依分期随机分为同期放化疗(实验组)和单纯放疗(对照组)。B点46～50Gy/23～25f,A点40～48Gy/10～12f。同期化疗采用DDP40mg.d1,每周重复。结果:实验组总近期有效率、1年生存率和3年生存率较对照组有明显的提高趋势(P<0.05)。3期患者CR+PR,实验组和对照组分别为82.35%和50.00%,差异有统计学意义(P<0.05)。实验组2期患者1年局控率与3期患者3年生存率较对照组差异有统计学意义(P<0.05)。所有患者治疗后体力状况和生活质量均有明显改善。腹泻和血液毒性为该治疗模式的限制性毒性(P<0.05)。结论:同期放化疗联合辅助化疗可以提高中晚期宫颈癌患者3年生存率和局控率。体力和生活质量较治疗前提高,限制性毒性经处理可以耐受。Objective：To observe the effect concurrent radiochemotherapy combined adjunct chemotherapy on advanced cervical cancer.Methods：A total of 138 cases with cervical squamous cell carcinoma atⅡa ～Ⅲb stage were randomly divided into the concurrent chemoradiotherapy group（experimental group）and radiotherapy group（control group）.Experimental group were treated by intravenous drip of DDP 40 mg/dL,repeated every week.Radiotherapy group was administered by intracavity and external beam radiation therapy.Point A was given by 40 48 Gy/1012 f and point B by 4650 Gy/2325 f.Results：Short-term efficiacy,one-year and 3-year survival rate were significantly higher in experimental group.There were significant differences in local control rate of patients at stageⅡand 3-year survival rate of patients at stage Ⅲ（P〈0.05）.Physical condition and quality of life of all patients showed significant improvement after treatment.Diarrhea and blood toxicity were found to be limited toxicity of the treatment.Conclusions：Concurrent chemoradiotherapy combined chemotherapy can increase the short-term efficacy,1-year and 3-years survival rate and local control rate of patients with advanced cervical cancer,improve physical status and quality of life,and promote the tolerance of limited toxicity.
