某县级医院1726例消化性溃疡胃镜分析

摘　　要：目的:了解某县溃疡病流行病学特点。方法:对某县医院2004年1月～2009年12月6年间经胃镜检查诊断为消化性溃疡(PU)的病例资料进行回顾分析。结果:6年间胃镜检查11588例,检出PU1726例(14.89%),十二指肠球部溃疡(DU)1108例,胃溃疡(GU)542例,复合性溃疡(CU)76例;DU和GU检出率男性高于女性(P<0.01),DU检出率40岁以下组高于40岁以上组(P<0.01),GU和CU结果相反;城镇人群PU检出率低于乡村(P<0.01),2007～2009年度段PU检出率低于2004～2006年度段(P<0.01)。结论:PU好发部位以DU最多见,GU次之,CU最少,DU发病年龄较GU早;溃疡患病率男性高于女性,乡村高于城镇,有逐年降低趋势。Objective：To investigate the epidemiology characters of peptic ulcer in the study area.Methods：Data of peptic ulcer（PU）patients that confirmed by endoscopy during January 2004 December 2009 were retrospectively analyzed.Results：A total of 11 588 patients underwent endoscopy examination,of which 1 726 cases were confirmed as PU（14.89%）,1 108 were duodenal ulcer（DU）,542 were gastric ulcer（GU）,and 76 were complex ulcer（CU）;The prevalence of both DU and GU were higher among male than female（P〈0.01）,DU detection rate among those below 40 years of age was higher than those above 40 years old（P〈0.01）,while for GU and CU,opposite results were shown.Prevalence of PU was lower among urban residents than rural ones（P〈0.01）,PU was less popular during 2007-2009,comparing with the period of 2004-2006（P〈0.01）.Conclusion：DU is the most common PU,followed by GU and CU.Besides,age of DU onset is younger than GU.PU prevalence is higher among male than female,rural than urban,showing a decrease tendency year by year.
