心理干预对中青年乳腺癌根治术后患者心理状态及生活质量的影响

摘　　要：[目的]探讨心理干预中青年乳腺癌根治术后患者心理状态及生活质量的影响.[方法]对86例中青年患者随机分为对照组和干预组各43例.对照组采取常规护理;干预组在常规护理的基础上,进行心理干预.采用焦虑自评量表（SAS）、抑郁自评量表（SDS）和生活质量核心问卷（EORTC QLQ-C30）以及适用于乳腺癌的特异性量表（EORTC QLQ-BR23）分别于术前和术后半年对2组患者进行评分.[结果] 两组患者在术前SDS、SAS评分比较差异无统计学意义（P〉0.05）,术后半年干预组SDS、SAS评分显著低于对照组（P〈0.01）,术后半年QLQ-C30量表中干预组所有功能量表分值增加,症状量表中疲乏、呼吸困难、食欲不振、失眠和疼痛分值均下降,与对照组相比有明显差异.[结论]心理干预能改善中青年乳腺癌根治术后患者的心理状态,提高生活质量.[Ohjective] To explore the effect of psychological intervention on the psychological status and the quality of life of young and middle-aged patients with breast cancer after mastectomy. [Methods] A total of 86 ca ses of young and middle-aged patients were randomly divided into two groups. The control group（ n = 43） was treated with routine care, while the intervention group（ n = 43） was treated with psychological intervention and health education before and after operation besides usual care. The self-rating anxiety scale（SAS）, self-rating depression seale（SDS）, EORTC QLQ-C30 and EORTC QLQ-BR23 were carried out before and 6 months after mastectomy. [Results] There were no significant differences between the groups in the scores of SDS and SAS before masteetomy（ P 〉0.05）, while the scores of SDS and SAS in the intervention group were significantly lower than those in the control group at 6 months after mastectomy （ P 〈0.01）. The scores of all functional scales in QLQ C30 scale in the intervention group at 6 months after masteetomy increased, while the scores of fatigue, dyspnea, anepithymia, insomnia and pain in symptom scale declined, and displayed a significant difference with the control group. [Conclusion] Psychological intervention can improve the psychological status and the quality of life among young and middle-aged patients with breast cancer after masteetomy.
