摘　　要：目的分析氨磷汀在肝动脉化疗栓塞术(TACE)中保护肝功能的作用。方法回顾性分析TACE治疗肝脏恶性肿瘤患者485例次临床资料,按是否使用氨磷汀分成观察组(254例次)和对照组(231例次),分别检测术前、术后4d和40d时总胆红素(TB)、谷氨酸转氨酶(ALT)、天门冬氨酸转氨酶(AST)和白蛋白(A)。结果两组TB、ALT及AST,术后4d均较术前显著增高,术后40d较术后4d显著降低(均P<0.01);A则术后4d显著低于术前及术后40d(均P<0.01)。术前与术后4d比较,观察组TB、AST及A差值与对照组均有显著差异(均P<0.01);术前与术后40d比较,两组各项指标均无明显差异(均P>0.05)。肝功能Child A级时,术前与术后4d比较,观察组TB和A差值与对照组均有显著差异(均P<0.01);ChildB级时,观察组TB、A、ALT和AST差值与对照组均有显著差异(均P<0.01)。结论 TACE中氨磷汀对肝功能有一定的保护作用,在肝功能较差时相对明显;但其保护作用有限,对长期肝功能的保护优势尚不明确。Objective To evaluate the role of amifostine in hepatic function protection after chemotherapy hepatic artery embolization（TACE）.Methods Four hundred and eighty five cases of hepatic malignant tumors treated by TACE were selected retrospectively,which were divided into the treatment group（254 cases） and the control group（231 cases） according to the usage of amifostine.Total bilirubin（TB）,ALT,AST and albumin were detected before TACE,4 days and 40 days after TACE respectively.Results Between before TACE and 4 days after TACE,the deviation values（DVs） of TB and AST in the treatment group were lower than that in the control group while A was on the contrary,which was significant（P〈0.01） ;between before TACE and 40 days after TACE,DVs in both groups were not significant.In hepatic function of Child A,DV of TB between before TACE and 4 days after TACE in the treatment group was lower than that in the control group（P〈0.01）,while A was on the contrary（P〈0.01） ;in Child B,DVs of all in treatment group were different significantly from those in the control group（P〈0.01）.Conclusion In TACE ammonia phosphoric christine has some protective effect on hepatic function,and the protective effect is more obvious in poor hepatic function;but for now the protective effect is limited,and the superiority of long protective effect on hepatic function is uncertain.
