DF方案治疗30例老年晚期胃癌的疗效观察

摘　　要：目的观察多西他赛(DOC)联合5-氟尿嘧啶(5-Fu)和亚叶酸钙(CF)(简称DF方案)治疗老年晚期胃癌的临床疗效及不良反应。方法对30例老年晚期胃癌患者,采用DOC75mg/m2,静脉滴注,第1天;CF200mg/m2静脉滴注,第1天,2h后5-Fu500mg静脉推注,随后5-Fu2500mg/m2持续48h化疗泵静脉内注入,21天为1个周期,2周期后评价疗效。结果 30例患者均可评价疗效,其中完全缓解3例,部分缓解13例,稳定9例,进展5例,有效率53.3%,中位疾病进展时间为5.4个月,中位生存期10.8个月。不良反应主要是骨髓抑制、胃肠道反应和手足综合征,均为可逆性,无治疗相关性死亡。结论 DF方案治疗老年晚期胃癌疗效好,不良反应轻,患者耐受性好,值得临床推广。Objective To observe the clinical efficacy and adverse reactions of the combination of docetaxel （DOC）,Fluorouracil （5-Fu） and Calcium Folinate （CF） （DF project for short）in the treatment of elderly patients with advanced gastric cancer.Methods 30 patients with advanced gastric cancer were treated with DOC 75 mg/m2,CF 200 mg/m2 iv the first day,and 5-Fu 500mg iv after 2 hours,then injected 5-Fu 2500 mg/m2 through a vena pump continuously for 48 hours;repeated every 3 weeks （as a therapeutic cycle）,The effect was evaluated after two cycles.Results clinical response was evaluated in 30 patients.3 patients completely response,13 patients partially response,9 at the stable stage and 5 progression were observed with an overall response rate of 53.3%,The median time of progression was 5.4 months and median survival time were 10.8 months.The most common toxicities were bone marrow suppression,vomiting and Hand-foot-syndrome.All of them are reversible.There was no treatment related death.Conclusion Combining DOC with 5-Fu and CF is an effective and better tolerated alternative treatment on elderly patients with advanced gastric cancer and yields promising activity.
