两种拔牙方式矫治安氏Ⅱ类Ⅰ分类错的软硬组织变化

摘　　要：目的:研究拔除上下颌第一前磨牙或上颌第一前磨牙、下颌第二前磨牙矫治对青少年安氏II类Ⅰ分类错软硬组织的影响,为临床治疗提供参考。方法:选择拔除上下颌第一前磨牙(A组)或上颌第一前磨牙、下颌第二前磨牙(B组)矫治的青少年病例各10例。两组样本的性别、年龄、疗程相互匹配。对两组治疗前后的头颅侧位片进行测量分析,研究不同拔牙模式治疗前后软硬组织的变化。所有数据均采用SPSS11.5软件包进行统计学处理。结果:两组病例矫治后U1-L1,S-GO,N-ANS,ANS-Me增加,U1-SN减小;Holdaway软组织分析显示:两组病例矫治后上唇沟深,鼻下点至H线距,H角,下唇至H线距,颏唇沟深度减小,两组差别无统计学意义。结论:两种拔牙方式均能有效内收上切牙,改善软组织侧貌。下颌拔牙位置对软组织侧貌的改变无明显影响。Objective：To evaluate the lateral cephalometric changes of hard and soft tissues with orthodontic treatment involving the extraction of various premolars.Methods：pretreatment and posttreatment lateral cephalograms of 20 adolescent patients were collected,traced and digitized.Of these 20,10 involved the extraction of upper and lower first premolars,and 10 involved the extraction of upper first premolars and lower second premolars.Gender,age,and treatment time were matched in two groups.Results：In the two groups,U1-SN,upper lip depth,Sn-H,LL-H,and H angle reduced significantly,U1-L1,S-Go,N-ANS,ANS-Me increased significantly,but there was no significant difference between two groups.Conclusions： The convexity of soft and hard tissue was decreased with both extraction treatment.The site of mandibular extraction did not seem to have obvious effect on the changes of profile.
