摘 要:目的探讨硫酸镁(MgSO4)治疗对大鼠蛛网膜下腔出血(SAH)后急性脑血管痉挛(CVS)血中一氧化氮(NO)和内皮素-1(ET-1)水平的影响。方法将128只Wistar大鼠随机分为正常对照组、假手术组、单纯SAH组和SAH+MgSO4组。采用枕大池二次注血法制作大鼠SAH后急性CVS动物模型,分别于术前1h和术后1h、6h、24h采用显微图像分析仪测量各组基底动脉直径,用硝酸盐还原酶法测定各组血清NO水平,用放免法测定各组血浆ET-1水平。结果单纯SAH组在术后基底动脉直径缩小,SAH+MgSO4组在术后基底动脉直径大于单纯SAH组,但小于正常对照组和假手术组,差异均有统计学意义(P<0.05)。单纯SAH组术后血清NO水平较正常对照组和假手术组降低;血浆ET-1水平较正常对照组和假手术组升高,差异均有统计学意义(P<0.05)。SAH+MgSO4组术后血清NO水平较单纯SAH组升高,但低于正常对照组和假手术组;血浆ET-1水平较单纯SAH组降低,但高于正常对照组和假手术组,差异均有统计学意义(P<0.05)。结论血清NO水平和血浆ET-1水平的变化和动态平衡的破坏与SAH后急性CVS有关。MgSO4的治疗能够使血清NO、血浆ET-1水平恢复正常从而起到防治SAH后急性CVS的作用。Objective To discuss the effect of magnesium sulfate (MgSO4) on levels of nitric oxide (NO) and endothelin-1 (ET-1) in rats with cerebral vasospasm (CVS) following subarachnoid hemorrhage (SAH).Methods One hundred twenty-eight Wistar rats were divided randomly into groups A (normal control group),B (sham operation group),C (simple SAH group) and D (SAH+MgSO4 group).CVS models following SAH were established by 2-time blood injection in cisterna magna.Basilar artery diameter were determined by microscopic image analysis system,NO level by nitrate reductase method and ET-1 level by radioimmunoassay 1 h before operation and at hours 1,6,24 after operation.Results After operation,basilar artery diameter shrinked in group C;the basilar artery diameter was longer in group D than in group C,but shorter than in groups A and B,the difference was significant (P0.05).NO level was lower in group C than in groups A and B,ET-1 higher,the difference was significant (P0.05).NO level was higher in group D than in group C,but lower than in groups A,B,and ET-1 level lower than in group C,but higher than in groups A,B (P0.05).Conclusion The changes and dynamic equilibrium of serum NO and plasma ET-1 are related to acute CVS.MgSO4 can make levels of NO and ET-1 return to normal,thus to prevent post-SAH CVS.
