摘  要:目的检测膀胱移行细胞癌(BTCC)中基质金属蛋白酶-9(MMP-9)及血管内皮生长因子(VEGF)的表达,探讨其在BTCC发生、发展过程中的作用及其相互之间的关系。方法采用免疫组化S-P法检测61例BTCC和12例正常膀胱黏膜组织标本MMP-9和VEGF的表达水平,统计分析其与男女性别、病灶大小、病理分级、临床分期的相关性。结果 BTCC组织中与正常膀胱组织MMP-9及VEGF阳性表达率比较,差异有统计学意义(P<0.05)。男性及女性BTCC组织中MMP-9及VEGF阳性表达率比较,差异无统计学意义(P>0.05)。BTCC组织中>3cm肿瘤病灶MMP-9阳性表达率较≤3cm肿瘤病灶明显升高,差异有统计学意义(P<0.05)。BTCC组织中G3期MMP-9阳性表达率较G1~G2期明显升高,差异有统计学意义(P<0.05)。BTCC组织中T2~T4期MMP-9阳性表达率较Tis~T1期明显升高,差异有统计学意义(P<0.05)。结论 MMP-9及VEGF在BTCC的表达规律及其与肿瘤浸润、转移及与血管生成的密切相关,为BTCC的预后估计及靶向治疗提供了一个新的方法。Objective To explore the expressions of matrix metalloproteinase-9(MMP-9) and vascular endothelial growth factor(VEGF) in the bladder transitional cell carcinoma(BTCC) to find out their role in BTCC development and their relationship.Methods The expressions of MMP-9 and VEGF in 61 cases of BTCC bladder tissue specimens and 12 cases of normal were detected by immunohistochemical SP method,to find out their relationship to gender,tumor size,pathological grading and clinical staging.Results There was significant difference in MMP-9 and VEGF positive expression between BTCC bladder tissues and normal(P〈0.05),but not between BTCC tissues of males and females(P〉0.05).MMP-9 positive expression was higher in BTCC tissues of〉 3 cm tumors than in those of ≤3 cm(P〈0.05),higher in G3 stage than in stages G1~G2(P〈0.05),higher in stages T2~T4 than in stages Tis~T1(〈P0.05).Conclusion The expression regularity of MMP-9 and its mechanism closely related to tumor invasion,metastasis and angiogenesis of bladder cancer provide a new method for prognosis evaluation and targeted therapy of BTCC.
