带蒂全额皮瓣在晚期头颈肿瘤术后修复中的应用

摘　　要：目的：探讨带蒂全额皮瓣整复头颈部肿瘤术后组织缺损的可行性。方法：对１４例晚期头颈部肿瘤术后鼻面部、鼻咽部、软腭等部分组织缺损，取全额皮瓣予以整复，取瓣面积（５ｃｍ×１１ｃｍ）～（７ｃｍ×１６ｃｍ）。结果：皮瓣完全存活１２例，部分坏死２例，患者３年和５年生存率分别为７６．９％（１０／１３）和６３．６％（７／１１）。经鼻腔和口腔呼吸好，进食功能良好。结论：晚期头颈部肿瘤术后组织缺损应用全额皮瓣整复是一种较好的取材方法，修复面积大。Objective: To discuss the feasibility on reconstructing the defective tissue with the pedicle skin flap of forehead after headneck tumors dissection. Method: 14 patients with advanced headneck tumors were studied, whose defective tissues were reconstructed by their forehead skin flaps.The flap area ranged from 5 cm11 cm to 7 cm16 cm. Result: 2 cases of them were par|ial necrosis,3 years and 5 years survial rates were 76.9% and 63.6% respectively.The function of breath through nose or mouth and the function of swallow were satisfactory.Conclusion: The forehead skin flap should be a good selection for the reconstruction of defective tissue after basicranial and nasalfacial dissection.
