PICC置管在慢性呼吸衰竭患者中的临床应用研究

摘　　要：目的探讨经外周静脉穿刺的中心静脉置管(PICC)在慢性呼吸衰竭患者临床应用的效果,为临床急救的静脉输液方案提供依据。方法选择慢性呼吸衰竭患者100例,随机抽取30例为PICC组,70例为浅静脉穿刺对照组,比较两组静脉输液日均补液治疗时间(h/d)、不良反应的发生率和急救药物使用情况。结果两组患者在日平均补液治疗时间无明显差异(P>0.05);PICC组不良事件发生率为10%,对照组不良事件发生率为61.4%,明显高于PICC组(P<0.05);PICC组患者对静脉营养和血液制品方面的使用率明显高于对照组(P<0.05),在使用血管活性药物方面无明显差异(P>0.05),急救药物用药及时性及连续性方面比较,PICC组抢救次数为20人次,平均抢救及时用药时间为2.50±0.83min,对照组抢救次数为27人次,平均抢救及时用药时间为6.19±1.27min,PICC组用药及时性明显高于对照组。结论 PICC置管能够有效降低穿刺次数,保证补液治疗的连续性,提高特殊药物的使用安全性,有效降低静脉输液不良事件的发生率,同时还能够保证抢救时用药的及时性。Objective To compare the clinical application advantages and disadvantages of PICC catheter and shallow venipuncture catheter in chronic respiratory failure patients,for provides optimization scheme of clinical intravenous infusion. Methods To extract 30 chronic respiratory failure patients who have PICC catheter randomly in respiratory medicine part as experimental group,extract 70 cases who have shallow venipuncture catheter randomly as control group,comparing the daily intravenous infusion time（hours/days） and adverse reaction of two groups patients. Results Two group of patients in daily intravenous infusion time have no obvious difference（P 0.05）. The experimental group is obviously higher than control group（P 0.05） in the using of parenteral nutrition drug and obviously lower（P 0.05） in the adverse events happen rate. Conclusion Patients with chronic respiratory failure in clinical transfusion treatment,useing PICC catheter can not only effectively reduce the puncture and guarantee the continuity of the rehydration treatment,but also can improve the safety in the use of special drug and reduce happen rate of adverse events in intravenous infusion.
