121周期体外受精-胚胎移植临床分析

摘　　要：目的:总结分析内蒙古地区行体外受精-胚胎移植技术助孕治疗病人的临床特点及治疗效果,为今后的治疗提供临床资料及经验。方法:回顾性分析121周期体外受精-胚胎移植技术助孕治疗病人的临床资料,对比分析不同方案之间病人的年龄及病因有无区别,对比分析长方案、短方案这两个常用方案治疗结局有无区别。结果:总体临床妊娠率达39.6%,长方案与短方案两组在病因构成及不育年限上的区别无统计学意义。在不育女方的年龄上,短方案组(34±4.10岁)平均年龄比长方案组(31.3±3.88岁)大(P<0.05),两组妊娠率(长:33.7短:36.4)上没有显著区别,短方案(8.28±1.73)使用促性腺激素的天数比长方案(10.66±1.63)短,平均获卵数比长方案少(P<0.05),正常受精率比长方案少,但优质胚胎率(长:39.62±23.89;短:37.52±33.56)及种植率上两组没有区别。结论:体外受精-胚胎移植技术是治疗不育症的有效方法,长方案及短方案是临床上常用方案,个体化方案是临床工作的选择。Objective：To analyze the clinical feature and therapeutic effect of patients enrolled in controlled ovarian stimulation（COH） in vitro fertilization and embryo transfer （IVF -ET）cycle. Methods： To analyze retrospectivly the patients＇ s clinic information of 121 IVF - ET cycle, the age of patients and etiology in different groups were compared, the effect of IVF in long protocol group and short protocol group were compared. Results：Clinical pregnancy rate was 39.6% in our hospital, there was on different in the composition of etiological factor and infertility duration between the two group （P 〉 0.05）, patients were older in the short protocol group than in the long protocol group, there was fewer retrieved ovums in the short protocol group than those in the long protocol group, there was lower of 2pn rates in the short protocol group than those in the long protocol group （ P 〈 0.05 ）. there was on different in the pregnancy rates （ P 〉 0.05 ）, there was on different of higer quality embryos and implantation rates between the two group（ P 〉 0.05 ） ,patients in the short protocol group needde fewer Gn days than those in long protocol group. Conclusion： IVF - ET was the effective method in infertility treatment, the long and the short controlled ovarian hyperstimulation （COH）protocol were the classic protocol, Individuate protocol was nessisary.
