选择性支气管动脉灌注立止血治疗大咯血临床应用

摘　　要：目的探讨经导管选择性支气管动脉灌注立止血治疗大咯血的临床疗效。方法 98例大咯血患者,均采用Seldinger技术,明确诊断后,在DSA电视监视下,A组:29例行单纯立止血灌注,B组:38例行立止血+明胶海绵颗粒栓塞,C组:31例行立止血+Fe3O4微粒栓塞。结果 A组治愈率55.2%,B组治愈率89.5%,C组治愈率96.8%。结论经导管选择性支气管动脉灌注立止血+固体颗粒栓塞方法简便、安全可靠,能明显提高大咯血的成功率。Objective To observe the therapeutic effect of selective bronchial artery infusion with reptilase in the treatment of severe or repeated hemoptysis.Methods The embolization therapy in 98 cases using reptilase（A stage：29cases）,reptilase＋Gefoam（B stage：38 cases）,or reptilase ＋microspheres of Fe3O4（C stage：31cases）under TV guided through catheterization using seldingers technique were performed.Results A stage：The cure rate was 55.2%,B stage：The cure rate was 89.5%,C stage：The cure rate was 96.8%.Conclusion Bronchial artery Infusion-embolization therapy is a safe,simple and effective method,and has the good cure rate significantly.
