CT引导下冷循环微波刀治疗转移性肝癌的疗效分析

摘　　要：目的探讨CT引导下冷循环微波刀治疗转移性肝癌的安全性及有效性。方法选择236例转移性肝癌患者接受冷循环微波刀治疗,观察手术前后临床症状、肿瘤标记物及肝功能的变化,通过CT比较转移性肝癌大小及血流情况。以及术后1、2、3年生存率。结果肝功能于术后1周呈现一过性异常,1个月内获得改善。术前肿瘤直径为38.5±10.1mm,术后1个月减为27.6±7.6mmP<0.05;术后1,2,3年生存率分别为:72.1%,43.8%,20.0%。全组无严重并发症及手术死亡发生。结论冷循环微波刀治疗安全有效,操作简单,并发症少,符合微创治疗的理念,是手术不可切除转移性肝癌的理想治疗手段。Objective To explore the clinical value of CT guided psychro-circulation percutaneous microwave coagulation therapy PMCT in the treatment of hepatic metastasis.Methods Two hundred and thirty-six hepatic metastasis patients received psychro-circulation PMCT in our center from January 2007 to December 2008.The diameters and blood flow state of the tumors were measured and compared by use of computerized tomography.The postoperative 1,2 and 3 y survival rate of those patients was followed up and recorded.Results No mortality or severe complications occurred in this series.Preoperative tumor diameter was 38.5 ±10.1 mm and diminished to 27.56±7.6 mm at 1 month postoperatively（P0.05）.In the patients with secondary hepatic carcinoma,the postoperative 1,2 and 3 y survival rate was 72.1%,43.8%,and 20.0%.Conclusions Psychro-circulation PMCT is an effective method for treating hepaticmetastasis,and it is especially suitable for inoperable patients because of its minitrauma,few complications and convenient performance.
