老年性白内障小切口手术后早期低视力原因分析

摘　　要：目的:探讨老年白内障小切口手术后早期低视力的主要原因。方法:回顾性分析2009年7月至2010年6月在我院行白内障手术的1017眼术后1周视力低于0.3的102眼的眼部情况。102例患者中男50眼,女52眼,年龄为58至89岁,平均71.3岁,术前均常规进行视力、裂隙灯、眼压、角膜曲率、眼A/B超检查,人工晶体度数测算等检查,手术按标准的小切口无缝线手术完成,术中无严重并发症出现,术后1周,1月及3月时复查眼部及视力情况,分析术后短期内即1周时视力恢复不佳的原因。结果:低视力的患者即术后视力低于0.3的患者分析其原因主要有:术中出现轻度并发症导致术后短期内视力不佳的有8眼占7.84%;术后出现并发症的有7眼占6.76%;术前存在眼部疾病但因白内障遮挡不能被检查到的而在术后被发现的有87眼,占85.29%,87眼中有66眼患有眼底疾病占75.87%。结论:老年性白内障小切口手术后短期内低视力的主要原因为手术眼的原有眼部疾病。Objective：To study the main factors of low vision after small incision cataract surgery （SICS） in senile cataract patients. Methods SICS were performed in 1017 eyes from June 2009 to May 2010. Among 1017 patients, the clinical data of 102 eyes of 102 patients with best corrected visual acuity less than 0.3 at 1 week after surgery were analyzed. The 102 patients, 50 male and 52 female, aged 58 to 89 years and averaged 71.3 years old. Tile visual acuity, slit lamp, intraocular pressure, corneal curvature, ocular A / B ultrasound and IOL degree were examined before surgery. SICS was performed through scleral tunnel incision and no serious complications happened during surgery. Eye and vision conditions were examined at 1 week, 1 month and 3 months after surgery. The reasons of poor visual acuity at 1 week post surgery were analyzed. Results Eight eyes （7.84%） had the sliglit complication due to the complication during surgery, and seven eyes （6.76%） had postoperative complication. Among 102 eyes, 87 eyes had intrinsic eye disease （85.29 % ） before surgery, and 66 within 89 eyes （75.87 % ） had retinopathy after surgery. Conclusion Intrinsic eye disease, especially the retinopathy is the main factor of low vision after SICS at the senile cataract patients.
