椎管内胆脂瘤的诊断与治疗

摘　　要：对２７例经手术和病理证实的椎管内胆脂瘤患者的临床资料进行分析。发现患者的临床表现无特异性；ＣＴ检查示肿瘤呈均一低密度影，边界清楚，强化扫描多无增强；ＭＲ检查示肿瘤信号强度变化不定，边界清楚。２７例患者均行手术治疗，肿瘤全切６例、次全切除１７例、大部切除４例；术后对２３例随访０．５～７年，２例复发，均再次行手术治疗后好转。表明ＣＴ和ＭＲ检查有助于椎管内胆脂瘤的术前诊断，显微镜下全切是治疗本病的最好选择。The clinical datas of 27 patients with intraspinal epidermoid cyst which were confirmed by operation and pathologic examination were analysed The clinical presentation of the patients with this tumor is not specific On CT scans the 23 tumors were characterized by a homogencous low density lesion and well defined limits,they usually do not enchance with contrast medium On MR imaging the tumors were characterized by an important variability of signal intensity,well defined limits All patients underwent resections of the epidermoid cyst,6 cases total resections,17cases subtotal resections,4 cases gross partialresections 23 cases were followed up from half a year to 7 years 2 cases recured after the operation(4,7 years),second resections were performed The results indicate that CT scans and MR imagings are the effective methods of the preoperative diagnosis of this tumors,the tumors are not easy to be resected totaly The total resection of this tumor is possible by using microsurgery
