摘  要:目的探讨大肠腺瘤组织中脂及脂质过氧化与大肠腺瘤恶变的关系。方法病例来源于2007年1月—2009年12月在武汉大学人民医院进行结肠镜检查诊断为大肠腺瘤的120例患者,所有病例均在结肠镜下行高频电切除大肠腺瘤,取腺瘤组织匀浆后,检查组织中胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇和丙二醛含量。结果远侧腺瘤组和绒毛状腺瘤组组织中胆固醇和丙二醛高于近侧腺瘤组和管状+混合状腺瘤组,差异有统计学意义(P<0.05);各组男女之间组织中胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇和丙二醛含量比较,差异无统计意义(P>0.05)。结论大肠腺瘤组织中胆固醇含量和脂质过氧化增高是大肠腺瘤恶变的一个危险因素。[Objective]To study the relationship between the lipid and lipid peroxidation in the colorectal adenoma tissue and malignancy of colorectal adenoma.[Methods]120 patients with colorectal adenoma diagnosed by colonoscopy in Renmin Hospital of Wuhan University from January 2007 to December 2009 were selected. The electroexcision of colorectal adenoma under colonoscopy was conducted in all patients. The contents of cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol and malondialdehyde were tested in the adenoma tissue homogenate.[Results]The contents of cholesterol and malondialdehyde in the tissue of the distal adenoma group and the villous adenoma group were higher than that of the proximal adenoma group and villous + tubulovillous adenoma,the different was significant ( P 0. 05) . There was no significant different in the contents of cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol and malondialdehyde in tissues between male and female patients in each group.[Conclusion]The content of cholesterol and increase of lipid peroxidation in the colorectal adenoma tissue is the risk factor of malignancy of colorectal adenoma.
