摘 要:为探讨一氧化氮(NO)、内皮素(ET)及5-羟色胺(5-HT)引发偏头痛的机制及其临床意义,对68例偏头痛患者用分光光度法测定血浆NO、放射免疫法测定ET、荧光分光法测定5-HT含量。结果发现偏头痛发作期血浆NO、ET含量明显高于对照组(P<0.01),而5-HT含量明显低于对照组(P<0.01);先兆期、间歇期血浆NO含量与对照组比较无差异;先兆期血浆ET含量与对照组比较差异有显著性意义(P<0.05),间歇期ET与对照组比较差异无显著性意义;先兆期、间歇期血浆5-HT含量明显高于发作期(P<0.01)。结果提示,5-HT是偏头痛发病的基础,是NO、ET的诱导剂和激活物;NO是引起血管扩张,导致偏头痛发作的主要因素;ET可能是导致血管收缩,引起先兆症状的重要因子。To explore the machanism of migraine induced by nitric oxide (NO), endothelin (ET) and 5 hydroxytryptamine (5 HT), and their clinical significance, assays for plasma concentrations of NO with spectrotometry, ET with radioimmunoassay, 5 HT with fluorescence spectrotometry were performed in 68 cases of migraine. The results showed that in the attack stage of migraine, the concentrations of plasma NO and ET were remarkably higher than the control group ( P <0.01), while the concentration of 5 HT was markedly lower than the control group ( P <0.01). The concentrations of plasma NO in the aura stage and the interim stage had no difference from that in the control group; the concentration of plasma ET in aura stage had great difference from the control group ( P <0.05). ET in the interim stage had no difference from the control group. The concentration of plasma 5 HT in the aura and interim stages was remarkably higher than the attack stage ( P < 0.01). It was concluded that 5 HT was the basis of migraine attack and the inducer and activator of NO and ET. NO was the main factor to cause the vasodilation, resulting in migraine attack; ET might be the important factor to cause vasoconstriction, resulting in the portent symptoms.
