胰腺上皮样血管平滑肌脂肪瘤1例并文献复习

摘　　要：目的:探讨胰腺上皮样血管平滑肌脂肪瘤(EAML)的临床病理学特点。方法:回顾复习1例胰腺EAML的临床、影像学资料,并进行组织病理、免疫组织化学分析及相关文献的复习。结果:切片镜下,肿瘤细胞为上皮样或梭形,胞浆丰富,透明或颗粒状,嗜酸性,呈片状或巢状分布,围绕间质中丰富的薄壁窦性血管形成器官样结构,可见少量厚壁血管,部分区域脂肪组织散在分布,背景中可见大量炎细胞浸润。免疫表型:肿瘤细胞HMB45阳性、Vimentin阳性,SMA及CD68散在阳性,CD34血管阳性,CD117、Desmin、S-100、CgA及AE1/AE3均阴性。结论:胰腺EAML是一种少见的间叶性肿瘤,容易误诊,需于胰腺内分泌肿瘤、副节瘤等肿瘤相鉴别,免疫组织化学染色是诊断及鉴别诊断的重要方法。Objective：To explore the clinicopathologic features of epithelioid angiomyolipoma（EAML） of pancreas.Methods：The clinical and imaging data in one case of EAML of pancreas were retrospectively analyzed with light microscopy and immunohistochemical staining.The related literatures were reviewed.Results：The patient was a 67 years old woman.Histologically,the tumor cells were epithelioid or spindle-like,with abundant clear or granular eosinophilic cytoplasm,and were arranged in sheets or nests around the vascular network.The abundant thin-wall sinus vessels around mesenchyma formed organ-like structure.A few thick-wall vessels were existed.Fat tissues scattered in partial area.Tumor contained plenty of inflammatory cells in the background.Immunohistochemical staining showed that the tumor cells were positive for HMB45,Vimentin,SMA and CD68,and negative for CD117,Dersmin,S-100,CgA and AE1/AE3.Vessels were expressed byCD34.Conclusion：EAML of pancreas is a rare mesenchymal tumor and easily misdiagnosed with other tumors.It needs to be differentiated from pancreatic endocrine tumors,paraganglioma and so on.Immunohistochemical staining is the important method for diagnosis and differential diagnosis of EAML.
