摘 要:目的观察穴位埋线结合强肝消脂饮对非酒精性脂肪肝(NAFLD)患者血清瘦素水平及胰岛素抵抗指数的影响。方法将100例患者随机分为穴位埋线结合强肝消脂饮治疗组(A组)、穴位埋线对照组40例(B组)、强肝消脂饮对照组40例(C组)、多烯磷脂酰胆碱胶囊对照组(D组)。各组患者治疗前后分别检测肝功能(ALT、AST)、血脂(CHOL、TG)、血清瘦素、空腹血糖及胰岛素、肝脏CT,并计算胰岛素抵抗指数。结果治疗组ALT、AST明显下降,优于各对照组;与此同时,治疗组CHOL、TG的下降亦优于各对照组。结论穴位埋线结合强肝消脂饮治疗可通过降低血清瘦素水平、改善胰岛素抵抗,增加肝细胞对胰岛素的敏感性,从而调节血脂代谢,达到治疗NAFLD的作用。Objective:To observe the effects of the acupoint catgut embedding and Qiangganxiaozhi Decoction on serum leptin and insulin resistance of the patients with nonalcoholic fatty liver disease(NAFLD).Methods: 100 pa-tients were randomly divided into three groups: the A group with acupoint catgut embedding and Qiangganxiaozhi Decoction,the B group with acupoint catgut embedding group,the C group with Qiangganxiaozhi Decoction and the D group with polyene phosphatidyl choline capsule.The contents including ALT,AST,CHOL,TG,LP,FBG and INS in serum of all patients were measured before and after treatment.Meanwhile,the variation of liver CT image were compared.And then,the HOMA-IR was calculated by these indices.Results: At the end of the treatment,these contents such as ALT and AST were decreased significantly compared with those before therapy.Also,there were obvious differences in these decrease.These levels including CHOL and TG after treatment were lower remarkably than those before therapy.The insulin resistance and the serum leptin were improved obviously by the acupoint catgut embedding and Qiangganxiaozhi Decoction.This improvement in the A group was significant superior to that in the other group.Conclusion: The serum leptin level could be reduced by the combination therapy.The insulin resistance is improved to increase the sensitivity of hepatocyte to the insulin.This effect could improve the blood lipid metabolism of NAFLD patients.
