双侧末端游离义齿修复前后髁状突位置三维CT观察

摘　　要：目的:通过三维CT(3D.X-CT)观测和评价双侧游离牙列缺损患者可摘局部义齿(RPD)修复前后颞下颌关节结构的变化。方法:对19例双侧游离牙列缺损患者进行RPD修复,分别于治疗前后拍摄双侧颞下颌关节(TMJ)CT片,进行统计学t检验比较关节间隙的线性测量数值。结果:颞下颌关节前间隙、后间隙在RPD修复前后相比,线性测量改变值有显著性差异(P<0.05);前、后间隙比值较治疗前明显缩小(P<0.05)。患者主诉颞下颌关节区不适症状缓解,面下1/3距离得以改善,TMJ髁状突位置基本居中。结论:三维牙CT的应用使颞下颌关节结构的变化得以量化,能客观评价双侧游离缺失患者通过RPD修复,改善双侧游离牙列缺损导致的垂直距离降低,TMJ髁状突前移回复于正常关节位,恢复了、肌肉、关节的协调关系临床疗效。Objective：To observe and evaluate the effect of the restoration of RPD for the patients with bilateral distal extention on the condyles position by three-dimensional dental CT（3D.X-CT）.Methods：19cases of bilateral distal extention were treated with RPD.Bilateral 3D.X-CT of TMJ at intercuspal position were taken before and after treatment,respectively.The TMJ spaces were measured and statistically analysed.Results：The linear distances of TMJ spaces show significant difference before and after treatment（P〈0.05）.The ratio between anterior and posterior TMJ spaces showed significant differences compared to pretreatment（P〈0.05）.The condyles position were in centric.The symptoms of TMDs and the paterns of face were changed.Conclusion：The restoration of RPD to the patients with bilateral distal extention could raise their OVD,and make the condyle retrude position in stable.It can also recover the harmony relationship among occlusion,muscles and TMJ.Meanwhile,application of 3D.X-CT can directly get quantitative change of TMJ.
